摘要
目的研究便携式高频超声在多发性肋骨骨折术中的定位价值。方法回顾性分析外伤后肋骨多发性骨折患者84例,并依据手术定位方式将其分为体表触诊定位、高频超声定位和胸腔镜定位三组。比较三种方法定位肋骨骨折准确性,并分析患者手术切口长度、手术时间、术中出血量、术后首日引流量、总引流量、术后3 d内视觉模拟评分法(VAS)评分、胸引管留置时间及住院天数等指标。结果高频超声定位组和胸腔镜定位组定位准确率明显高于体表触诊组(P<0.05)。高频超声定位组与胸腔镜定位组患者的手术时间、切口长度、术中出血量、术后首日引流量及总引流量、术后3 d内VAS评分、胸引管留置时间及住院天数明显低于体表触诊定位组(P<0.05),高频超声定位组手术时间短于胸腔镜定位组(P<0.05)。结论便携式高频超声能够精准定位多发性肋骨骨折部位,在手术时间和预后方面具有良好效果,有较高的实用价值。
Objective To evaluate the value of portable high-frequency ultrasound in the localization of multiple rib fractures during surgery.Methods A retrospective analysis was conducted on 84 patients with multiple post-traumatic rib fractures,who were divided into three groups based on the surgical localization method:palpation,high-frequency ultrasound,and thoracoscopy.The accuracy of rib fracture localization using these three methods was compared,along with the analysis of various surgical parameters in-cluding incision length,operation time,intraoperative blood loss,postoperative first-day drainage,total drainage,visual analog scale(VAS)score within 3 days post-operation,duration of chest tube placement,and hospital stay.Results The accuracy of localization in the high-frequency ultrasound group and thoracoscopy group was significantly higher than that the palpation group(P<0.05).The operation time,incision length,intraoperative blood loss,postoperative first-day drainage,total drainage,VAS scores within 3 days postoperation,duration of chest tube placement,and hospital stay in the high-frequency ultrasound group and the thoracoscopy group were significantly lower than those in the palpation group(P<0.05),with the operation time in the high-frequency ultrasound group being shorter than that in the thoracoscopy group(P<0.05).Conclusion Portable high-frequency ultrasound can accurate locate multiple rib fractures,showing good outcomes in operation time and prognosis,and has a high practical value.
