聚乙烯醇或玻璃酸钠联合环孢素在白内障术后干眼症中的效果

Effect of Polyvinyl Alcohol or Sodium Hyaluronate Combined with Cyclosporine in Dry Eye after Cataract Surgery

目的:探讨聚乙烯醇或玻璃酸钠联合环孢素在白内障术后干眼症中的效果。方法:选取2022年1月—2024年1月山东第二医科大学附属医院收治的120例白内障术后干眼症患者作为观察对象,采用随机数表法将患者分为A组(58例,聚乙烯醇联合环孢素)与B组(62例,玻璃酸钠联合环孢素)。比较两组治疗效果、角膜表面损伤和炎症程度、泪液膜稳定性、泪液分泌试验、角膜内皮细胞状态。结果:B组总缓解率为95.16%,高于A组的82.76%,差异有统计学意义(P<0.05);用药后,两组角膜荧光染色评分法评分低于用药前,且A组低于B组,差异有统计学意义(P<0.05);B组术后7 d、1个月泪膜破裂时间长于A组,差异有统计学意义(P<0.05);用药后,两组泪液分泌试验结果高于用药前,但A组低于B组,差异有统计学意义(P<0.05);用药后,两组角膜内皮细胞面积变异系数(coefficient of variation of endothelial cell area,CV)、平均细胞面积、角膜内皮细胞密度(endothelial cell density,ECD)优于用药前,且A组优于B组,差异有统计学意义(P<0.05)。结论:聚乙烯醇或玻璃酸钠联合环孢素均能治疗白内障术后干眼症,玻璃酸钠联合环孢素润湿效果更显著,聚乙烯醇联合环孢素抗炎效果更显著。

Objective:To investigate the effect of Polyvinyl Alcohol or Sodium Hyaluronate combined with Cyclosporine in dry eye after cataract surgery.Method:A total of 120 patients with dry eye after cataract surgery who admitted to Affiliated Hospital of Shandong Second Medical University from January 2022 to January 2024 were selected as observation objects,the patients were divided into group A(58 cases,Polyvinyl Alcohol combined with Cyclosporine)and group B(62 cases,Sodium Hyaluronate combined with Cyclosporine)by random number table method.The therapeutic effect,corneal surface damage and inflammation,tear film stability,tear secretion test and corneal endothelial cell status were compared between two groups.Result:The total remission rate of group B was 95.16%,which was higher than 82.76%of group A,and the difference was statistically significant(P<0.05);after medication,the corneal fluorescence staining scores of two groups were lower than those before treatment,and that of group A was lower than that of group B,the differences were statistically significant(P<0.05);the tear film rupture time of group B at 7 d and 1 month after operation was longer than that of group A,and the differences were statistically significant(P<0.05);after medication,the results of tear secretion test in two groups were higher than those before treatment,but the result in group A was lower than that in group B,and the differences were statistically significant(P<0.05);after medication,the coefficient of variation of endothelial cell area(CV),average cell area and endothelial cell density(ECD)in two groups were better than those before treatment,and those in the group A were better than the group B,the differences were statistically significant(P<0.05).Conclusion:Both Polyvinyl Alcohol or Sodium Hyaluronate combined with Cyclosporine can treat dry eye after cataract surgery,the wetting effect of Sodium Hyaluronate combined with Cyclosporine is more significant,and the anti-inflammatory effect of Polyvinyl Alcohol combined with Cyclosporine is more significant.
