奥氮平与利培酮对精神分裂症患者的治疗效果及心肌酶的影响对比

Comparative the Effects of Olanzapine and Risperidone in the Treatment of Schizophrenia and the Effects on Myocardial Enzymes

目的:对精神分裂症治疗中应用奥氮平与利培酮的效果及心肌酶影响情况进行比较。方法:选取2021年10月—2022年10月淮安市新安医院接收的80例精神分裂症患者,按不同用药方案分作两组,40例奥氮平治疗者为A组,40例利培酮治疗者为B组。对两组的症状改善情况、总有效率、药物副作用及心肌酶进行对比。结果:A组、B组治疗后的阳性症状与阴性症状量表(PANSS)评分对比,差异无统计学意义(P>0.05)。A组、B组总有效率分别是92.50%、95.00%,两组比较,差异无统计学意义(P>0.05)。A组药物副反应量表(TESS)中心血管系统评分高于B组,差异有统计学意义(P<0.05),两组TESS其余项目评分比较,差异无统计学意义(P>0.05)。A组治疗后的肌酸激酶同工酶(CK)、肌酸激酶同工酶(CKMB)高于B组,差异有统计学意义(P<0.05)。结论:在精神分裂症治疗中,奥氮平、利培酮均可对相关症状进行有效改善,疗效相当,但奥氮平对心血管产生的影响更明显,可导致心肌酶水平上升,临床上应依据患者实际情况合理对两种药物进行选择。

Objective:To compare the effects of Olanzapine and Risperidone in the treatment of schizophrenia and the effects on myocardial enzymes.Method:From October 2021 to October 2022,80 cases of patients with schizophrenia were selected from Huai’an Xin’an Hospital,they were divided into two groups according to different medication regiments.40 cases who were treated with Olanzapine as group A and 40 cases who were treated with Risperidone as group B.The symptom improvement,total effective rate,drug side effects and myocardial enzymes were compared between the two groups.Result:There was no significant difference in positive and negative syndrome scale(PANSS)scores between group A and group B after treatment,the difference was not statistically significant(P>0.05).The total effective rate of group A and group B was 92.50%and 95.00%,respectively,the difference was not statistically significant(P>0.05).The scores of the cardiovascular system of treatment emergent symptom scale(TESS)in group A were higher than those in group B,the difference was statistically significant(P<0.05),and the scores of other items in TESS between the two groups were compared,the difference was not statistically significant(P>0.05).Creatine kinase isoenzyme(CK)and creatine kinase isoenzyme(CKMB)in group A were higher than those in group B after treatment,the difference was statistically significant(P<0.05).Conclusion:In the treatment of schizophrenia,both Olanzapine and Risperidone can effectively improve related symptoms,with similar efficacy,but Olanzapine has a more obvious effect on myocardium,which can lead to the increase of myocardial enzyme levels.Clinically,the two drugs should be rationally selected according to the actual situation of patients.
