PaO^(2)、血乳酸水平预测急性呼吸衰竭并发多器官障碍的价值

摘要 目的:探讨动脉血氧分压(PaO^(2))、血乳酸水平预测急性呼吸衰竭并发多器官障碍价值。方法:选取医院2020年5月~2022年12月收治的189例急性呼吸衰竭患者,分为发生组和未发生组,分析入院时PaO^(2)、血乳酸水平与多器官功能障碍关系,用ROC预测其预测价值。结果:189例受试者,多器官功能障碍发生率25.93%。发生组PaO^(2)低(P<0.05),血乳酸水平高(P<0.05)。Logistic回归:PaO^(2)、血乳酸是独立危险因素(P<0.05)。ROC:PaO^(2)与血乳酸联合预测AUC为0.988,灵敏度95.90%,特异度96.40%。结论:急性呼吸衰竭并发多器官障碍与PaO^(2)、血乳酸水平密切相关,具较高预测价值。 Objective:To explore the value of arterial partial pressure of oxygen(PaO^(2))and blood lactate levels in predicting multiple organ dysfunction in acute respiratory failure.Method:189 patients with acute respiratory failure admitted to the hospital from May 2020 to December 2022 were selected and divided into an incidence group and a non incidence group.The relationship between PaO^(2),blood lactate levels,and multiple organ dysfunction at admission was analyzed,and the predictive value was predicted using ROC.Result:Among 189 subjects,the incidence of multiple organ dysfunction was 25.93%.The occurrence group had low PaO^(2)(P<0.05)and high blood lactate levels(P<0.05).Logistic regression:PaO^(2) and blood lactate were independent risk factors(P<0.05).ROC:PaO^(2) combined with blood lactate predicted an AUC of 0.988,with a sensitivity of 95.90%and a specificity of 96.40%.Conclusion:Acute respiratory failure complicated with multiple organ dysfunction is closely related to PaO^(2) and blood lactate levels,and has high predictive value.
