摘要
目的对医院门诊西药房中不合格处方点评情况进行分析,并总结其原因。方法回顾性分析本院门诊西药房开出的5000张处方单,对其中不合格处方加以点评,并归纳总结出其不合格原因。结果本次收集的5000张处方单中,不合格处方有103张,不合格率为2.06%,其中,不合理处方47张,占45.63%(47/103),不适宜处方39张,占37.86%(39/103),超常处方17张,占16.50%(17/103)。结论造成临床门诊西药房出现不合格处方的原因是多样的,在临床工作实践中需从多方面入手,严格规范医师行为,落实各项对账制度,做好处方点评工作,以此来减少不合格处方的出现。
Objective To analyze the unqualified prescription comments in outpatient pharmacies of hospital,and summarize their causes.Methods The 5000 prescriptions issued by outpatient pharmacy were retrospectively analyzed,the unqualified prescriptions were reviewed,and the reasons for the unqualified prescriptions were summarized.Results Among the 5000 prescriptions,103 unqualified prescriptions were reviewed,accounting for 2.06%,including 47 irrational prescriptions,accounting for 45.63%(47/103),39 inappropriate prescriptions,accounting 37.86%(39/103),17 extraordinary prescriptions,accounting for 16.50%(17/103).Conclusion There are various reasons for unqualified prescriptions in clinical outpatient pharmacies.In clinical work practice,it is necessary to start from many aspects,strictly regulate physicians'behaviors,implement various reconciliation systems,and do a good job in prescription comments to reduce the occurrence of unqualified prescriptions.
