摘要
目的探讨针对重症支原体肺炎患儿,应用阿奇霉素与小剂量(2 mg/kg)甲泼尼龙琥珀酸钠的价值。方法非随机选取2022年1月—2023年12月新沂市人民医院收治的100例重症支原体肺炎患儿为研究对象,按治疗方法不同分为对照组与联合组,每组50例。对照组使用阿奇霉素治疗,联合组在此基础上增加小剂量(2 mg/kg)甲泼尼龙琥珀酸钠,对比两组的病情改善时间,血清炎症指标[白细胞介素6(interleukin 6,IL-6)、C反应蛋白(C-reactive protein,CRP)、白细胞介素8(interleukin 8,IL-8)]、免疫指标。结果联合组白细胞计数恢复正常、体温恢复正常、咳嗽缓解、肺啰音消失及肺部阴影消失时间均短于对照组,差异有统计学意义(P均<0.05)。联合组的IL-6为(17.15±2.16)ng/L、CRP为(13.48±3.64)mg/L、IL-8为(24.81±2.94)pg/mL,优于对照组的(20.23±2.79)ng/L、(18.25±4.37)mg/L、(29.27±3.30)pg/mL,差异有统计学意义(t=6.172、5.930、7.136,P均<0.05)。联合组免疫指标低于对照组,差异有统计学意义(P均<0.05)。结论给予重症支原体肺炎患儿小剂量甲泼尼龙琥珀酸钠与阿奇霉素联合应用能加速病情改善,降低炎症反应,促使免疫功能改善,治疗效果较优。
Objective To investigate the value of azithromycin and low dose(2 mg/kg)methylprednisolone sodium suc-cinate in children with severe mycoplasma pneumonia.Methods 100 children with severe mycoplasma pneumonia treated in Xinyi People's Hospital from January 2022 to December 2023 were non-randomly selected as the study ob-jects,and were divided into control group and combination group according to different treatment methods,with 50 cases in each group.The control group was treated with azithromycin,and the combination group was supplemented with a small dose(2 mg/kg)of methylprednisolone sodium succinate.The improvement time of the two groups was com-pared.Serum inflammatory markers[interleukin 6(IL-6),C-reactive protein(CRP),interleukin 8(IL-8)]and immune markers.Results The time of white blood cell count,body temperature,cough relief,lung rales disappearance and lung shadow disappearance in combination group were shorter than those in control group,and the differences were statistically significant(all P<0.05).In the combined group,IL-6 was(17.15±2.16)ng/L,CRP was(13.48±3.64)mg/L and IL-8 was(24.81±2.94)pg/mL,it was better than(20.23±2.79)ng/L,(18.25±4.37)mg/L,(29.27±3.30)pg/mL in the control group,and the differences were statistically significant(t=6.172,5.930,7.136,all P<0.05).The immune indexes of the combined group were lower than those of the control group,and the differences were statistically signifi-cant(all P<0.05).Conclusion The combination of methylprednisolone sodium succinate and azithromycin in children with severe mycoplasma pneumonia can accelerate the improvement of the disease,reduce the inflammatory response,and promote the improvement of immune function.
