DCE-MRI在年轻女性乳腺肿瘤良恶性鉴别诊断中的应用价值分析

摘要 目的分析动态增强磁共振成像(DCE-MRI)对年轻女性乳腺肿瘤良恶性的鉴别诊断价值。方法98例年轻女性乳腺病变患者均行DCE-MRI检查,以病理诊断结果作为金标准,分析DCE-MRI鉴别诊断乳腺肿瘤良恶性的敏感度、特异度及准确度;根据病理学结果分组,比较乳腺良恶性肿瘤的DCE-MRI参数。结果病理诊断结果显示,98例乳腺病变患者共检出乳腺结节127个,包括良性结节88个,恶性结节39个。DCE-MRI鉴别诊断年轻女性乳腺肿瘤良恶性的敏感度、特异度及准确度分别为96.59%、53.85%、83.46%。良性病变组的病灶SER低于恶性病变组,Tmax、PER、ADC值高于恶性病变组(P<0.05);但两组的病灶EER比较,差异无统计学意义(P>0.05)。结论DCE-MRI对年轻女性乳腺肿瘤良恶性的鉴别有一定的价值,可通过Tmax、PER、ADC等参数对病变情况进行定性诊断。 Objective To analyze the value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in the differential diagnosis of benign and malignant breast tumors in young women.Methods 98 young female patients with breast lesions were examined by DCE-MRI.The sensitivity,specificity and accuracy of DCE-MRI in the differential diagnosis of benign and malignant breast tumors were analyzed with pathological diagnosis as the gold standard.According to the pathological results,the DCE-MRI parameters of benign and malignant breast tumors were compared.Results The results of pathological diagnosis showed that 127 breast nodules were detected in 98 patients with breast lesions,including 88 benign nodules and 39 malignant nodules.The sensitivity,specificity and accuracy of DCE-MRI in differential diagnosis of benign and malignant breast tumors in young women were 96.59%,53.85%and 83.46%,respectively.The SER of the benign lesion group was lower than that of the malignant lesion group,and the Tmax,PER and ADC value were higher than those of the malignant lesion group(P<0.05).Conclusions DCE-MRI has certain value in the differential diagnosis of benign and malignant breast tumors in young women.The lesions can be qualitatively diagnosed by parameters such as Tmax,PER and ADC.
