摘要
目的探讨初次人工膝关节置换术中使用压力传感导航技术对软组织平衡的作用效果。方法随机选取2021年9月至2022年12月南阳市第一人民医院骨二科收治的拟接受初次人工膝关节置换手术(TKA)的63例患者,按治疗方式分为治疗组(33例)和对照组(30例)。治疗组术中接受压力传感导航技术,在术中正确截骨的基础上,对膝关节周围的软组织张力进行精确平衡;对照组术中接受传统的关节平衡方法获得中立位力线和软组织平衡。对比两组手术时间、围手术期出血量、疼痛视觉模拟评分(VAS)、患侧膝关节活动度(ROM)、美国特种外科医院膝关节功能(HSS)评分、并发症发生情况。结果治疗组和对照组手术时间差异无统计学意义(P>0.05),治疗组围手术期出血量少于对照组(P<0.05);术后6个月治疗组ROM、HSS评分高于对照组,术后VAS评分低于对照组(P<0.05);治疗组术后恢复优等率占比高于对照组(P<0.05);两组术后不良反应发生率差异无统计学意义(P>0.05)。结论压力传感导航技术应用于初次人工膝关节置换术的软组织平衡,能够精确建立膝关节的稳定性,更好地恢复膝关节功能以及活动度,提高患者满意度。
Objective To investigate the impact of pressure sensor navigation on soft tissue balance in primary total knee arthroplasty(TKA).Methods A total of 63 patients undergoing primary TKA at the Second Department of Orthopedics,the First People’s Hospital of Nanyang from September 2021 to December 2022 were randomly selected and divided into treatment group(33 cases)and control group(30 cases)according to the treatment method.The treatment group received pressure sensing navigation technology during surgery,and on the basis of correct intraoperative osteotomy,the soft tissue tension around the knee joint was accurately balanced.The control group received traditional joint balance methods during surgery to obtain neutral force lines and soft tissue balance.The surgical time,perioperative bleeding volume,pain visual analogue scale(VAS),affected knee joint range of motion(ROM),knee joint function score from the US Hospital of Special Surgery(HSS),and incidence of complications between two groups were compared.Results There was no statistically significant difference in surgical time between the treatment group and the control group(P>0.05),and the perioperative bleeding in the treatment group was less than that in the control group(P<0.05).After 6 months of surgery,the ROM and HSS scores of the treatment group were higher than those of the control group,while the postoperative VAS scores were lower than those of the control group(P<0.05).The proportion of excellent postoperative recovery in the treatment group was higher than that in the control group(P<0.05).There was no statistically difference in the incidence of postoperative adverse reactions between the two groups(P>0.05).Conclusion The application of pressure sensing navigation technology in soft tissue balance during initial artificial knee replacement surgery can accurately establish the stability of the knee joint,better restore knee joint function and range of motion,and improve patient satisfaction.
