天麻钩藤饮治疗视神经病变的效果

Effect of Tianma Gouteng Decoction in the Treatment of Optic Neuropathy

目的分析天麻钩藤饮治疗视神经病变的临床应用价值。方法于2020年1月至2023年9月在濮阳市第二人民医院收集病例展开研究,将168例视神经病变患者随机分为研究组(84例,接受天麻钩藤饮治疗)与对照组(84例,接受银杏达莫注射液治疗)。对比两组疗效、盘周神经纤维层厚度、视网膜神经节细胞复合体厚度、视觉诱发电位P100波振幅及潜时、视力水平、视野的丢失差异值、平均光敏感度、视野平均缺损。结果研究组总有效率较对照组高(P<0.05)。治疗后,研究组盘周神经纤维层厚度、视网膜神经节细胞复合体厚度较对照组低(P<0.05)。治疗后,研究组15’P100波振幅、60’P100波振幅较对照组高,15’P100波潜时、60’P100波潜时较对照组低(P<0.05)。治疗后,研究组视力水平较对照组高(P<0.05)。治疗后,研究组视野的丢失差异值、视野平均缺损较对照组低,平均光敏感度较对照组高(P<0.05)。结论天麻钩藤饮治疗视神经病变患者效果显著,能降低盘周神经纤维层、视网膜神经节细胞复合体厚度,可改善视觉诱发电位参数,并能改善视野和视力水平。

Objective To analyze the clinical application value of Tianma Gouteng Decoction in the treatment of optic neuropathy.Methods A study was conducted from January 2020 to September 2023 at the Second People’s Hospital of Puyang.A total of 168 patients with optic neuropathy were randomly divided into study group(84 cases,treated with Tianma Gouteng Decoction)and control group(84 cases,treated with ginkgo leaf extract and dipyridamole injection).The curative effect,peripapillary nerve fiber layer thickness,retinal ganglion cell complex thickness,amplitude and latency of visual evoked potential P100 wave,visual acuity level,difference in loss of visual field,average light sensitivity,and average defect of visual field were compared between the two groups.Results The total effective rate in the study group was higher than that in the control group(P<0.05).After treatment,the peripapillary nerve fiber layer thickness and retinal ganglion cell complex thickness in the study group were lower than those in the control group(P<0.05).After treatment,the 15’P100 wave amplitude and 60’P100 wave amplitude in the study group were higher than those in the control group,while the 15’P100 wave latency and 60’P100 wave latency were lower than those in the control group(P<0.05).After treatment,the visual acuity level in the study group was higher than that in the control group(P<0.05).After treatment,the difference in loss of visual field and average defect of visual field in the study group were lower than those in the control group,while the average light sensitivity was higher than that in the control group(P<0.05).Conclusion Tianma Gouteng Decoction has a significant effect on patients with optic neuropathy,which can reduce the peripapillary nerve fiber layer thickness and retinal ganglion cell complex thickness,improve visual evoked potential parameters,and improve visual field and visual acuity levels.
