摘要
目的 分析超声引导下星状神经节阻滞(U-SGB)治疗冠心病(CHD)合并失眠症的疗效。方法 选取CHD合并失眠症患者98例,随机均分为观察组(常规治疗+U-SGB)和对照组(常规治疗)。比较两组心电图结果、下丘脑-垂体-肾上腺素水平、睡眠质量和生活质量改善情况。结果 与干预前比较,干预后两组心率减慢,QT间期、QT离散度缩短(P<0.05);且心率和QT离散度观察组较对照组改善更为显著(P<0.05)。与干预前比较,干预后两组血清促肾上腺皮质激素、促肾上腺皮质释放激素、皮质醇、匹兹堡睡眠质量指数降低,世界卫生组织生活评定量表评分升高(P<0.05);且观察组较对照组改善更为显著(P<0.05)。结论 U-SGB治疗CHD合并失眠症患者疗效显著,可预防心律失常,改善下丘脑-垂体-肾上腺素水平、睡眠质量和生活质量。
Aim To analyze the efficacy of ultrasound-guided stellate ganglion block(U-SGB) in the treatment of coronary heart disease(CHD) complicated with insomnia.Methods 98 patients with CHD combined with insomnia were randomly divided into an observation group(conventional treatment+U-SGB) and a control group(conventional treatment).The electrocardiogram results,hypothalamic pituitary adrenaline levels,sleep quality,and quality of life improvement between two groups were compared.Results Compared with the situations before intervention,the heart rate of the two groups slowed down,and the QT interval and QT dispersion shortened(P<0.05) after intervention;The observation group showed more significant improvement in heart rate and QT dispersion compared with the control group(P<0.05).Compared with the situations before intervention,the adrenocorticotropic hormone(ACTH),corticotropin-releasing hormone(CRH),cortisol(Cor),and Pittsburgh sleep quality index(PSQI) indices of the two groups decreased,and the world health organization quality of life BREF(WHOQOL-BREF) score increased(P<0.05) after intervention;And the observation group showed more significant improvement than the control group(P<0.05).Conclusion U-SGB has a significant therapeutic effect on patients with CHD combined with insomnia,which can prevent arrhythmias and improve hypothalamic pituitary adrenaline levels,sleep quality,and quality of life.
