认知行为干预对甲状腺癌病干预效果的Meta分析

Meta analysis of the effect of cognitive behavioral intervention on thyroid cancer patients

目的系统评价认知行为干预疗法在临床中对甲状腺乳头状癌患者产生的消极情绪、生活质量等方面的影响。方法利用中文数据库:中国知网、万方、维普、中华生物医学文献数据库等和英文数据库如PubMed、web of science等内的认知行为干预研究和对甲状腺乳头状癌患者治疗中所使用的随机对照试验,以检索自建库开始至二零二二年八月的全部资料,分别由2位研究员对照Cochrane手册5.1版偏倚风险评估工具对文献进行质量评价,将研究的资料分别进行提取与整理。制定选择、剔除准则,对现有资料加以筛选,文献管理软件采用Endnote20版,将数据使用RevMan5.3版本软件进行分析。结果纳入文献9篇,共895例病人,其中449例接受认知行为干预,446例接受常规护理干预。Meta分析结果显示:认知行为干预后,甲状腺癌患者焦虑[MD=一2.41,95%Cl(-3.94,-0.89),P=0.002]、抑郁[MD=-3.19,95%CI(-4.17,-2.20),P<0.001]的情况明显改善、生活质量水平提高[MD=5.75,95%CI(4.67,6.82),P<0.001]。结论对甲状腺乳头状癌患者实施认知行为干预疗法可以缓解患者焦虑和抑郁等情绪,对患者的后续治疗提供心理支持,有效提高患者的生活质量。

Objective To conduct a meta-analysis of the effects of cognitive behavioural interventions on negative emotions and quality of life in patients with thyroid cancer.Methods Randomized Controlled Trial of the use of Cognitive Behavioral Therapy in patients with papillary thyroid cancer in CNKI、Wanfang Data、VIP、CBM、PubMed and Web of Science.The entire literature from the establishment of the library to August 2022 was searched,and two investigators evaluated and extracted data using the Cochrane Hand book for Systematic Reviews of Interventions 5.1.literature screening was carried out using Endnote20 literature management software according to inclusion and exclusion criteria,and RevMan5.3 software was used for analysis.Development of inclusion and exclusion criteria to screen the literatures.Results A total of 9 articles with a total of 895 patients were included,449 of whom received cognitive behavioral intervention,and 446 of whom received routine nursing intervention.The results of meta-analysis showed that:After cognitive behavioral intervention,anxiety[MD=-2.41,95%CI(-3.94,-0.89),P=0.002],depression[MD=-3.19,95%CI(-4.17,-2.20),P<0.001]and quality of life of thyroid cancer patients were significantly improved[MD=5.75,P<0.001,95%CI(4.67,6.82),P<0.001].Conclusion Cognitive behavioral intervention for patients with thyroid cancer can effectively reduce anxiety and depression,and improve the quality of life of patients.
