MDT医护一体化护理在高血压伴急性缺血性脑卒中患者中的应用

Application of MDT Integrated Medical and Nursing Care in Patients with Hypertension and Acute Ischemic Stroke

目的 观察在对高血压伴急性缺血性脑卒中患者护理过程中开展多学科协作团队(MDT)的医护一体化护理干预的价值。方法 在2021年6月至2023年6月邵武市立医院高血压伴急性缺血性脑卒中患者中选择84例为对象,按照数字表法随机排序划分对照组(42例,开展常规护理)和观察组(42例,开展MDT医护一体化护理干预)。针对两组患者血压、神经缺损评分(NIHSS评分)及生活自理能力(ADL评分)评估改善情况、并发症发生率进行对比。结果 收缩压、舒张压检测与对比:护理干预前两组指标无明显差异,护理后两组指标水平均降低,且观察组指标水平较对照组低(P <0.05)。神经功能缺损以及生活自理能力评分:护理前两组评分无明显差异,护理后观察组生活自理能力评分较对照组高,神经功能缺损评分较对照组低(P <0.05)。对比两组并发症发生率,观察组低于对照组(P <0.05)。结论 在对高血压伴急性缺血性脑卒中护理过程中开展MDT医护一体化护理干预,可以降低患者血压水平,提升患者生活自理能力,减轻神经功能缺损程度,降低并发症发生率,有助于患者恢复。

Objective Observation of the value of multidisciplinary collaborative team(MDT) integrated nursing intervention in the nursing process of patients with hypertension and acute ischemic stroke.Methods Eighty-four patients with hypertension and acute ischemic stroke at Shaowu Municipal Hospital from June 2021 to June 2023 were selected as the subjects and randomly divided into a control group(42 cases,receiving routine nursing) and an observation group(42 cases,receiving MDT integrated nursing intervention) according to the numerical table method.Compare the improvement and incidence of complications in blood pressure,NIHSS score,and ADL score assessments between two groups of patients.Results The data showed that there was no significant difference in the indicators of systolic and diastolic blood pressure between the two groups before nursing intervention,but both groups decreased after nursing intervention,and the observation group was lower than the control group(P<0.05).There was no significant difference in scores for neurological deficits and self-care ability between the two groups before intervention.After nursing,the self-care ability score of the observation group was higher than that of the control group,while the neurological deficits score was lower than that of the control group(P<0.05).Comparing the incidence of postoperative complications between the two groups,the observation group was lower than the control group(P<0.05).Conclusions Carrying out MDT integrated nursing intervention in the nursing process of hypertension with acute ischemic stroke can lower patients' blood pressure levels,improve their limb and cognitive functions,reduce the incidence of complications,and help patients recover.
