宫腹腔镜联合治疗剖宫产瘢痕憩室的效果研究

Study on the Effect of Combined Hysteroscopy and Laparoscopy in the Treatment of Cesarean Scar Diverticulum

目的 分析宫腹腔镜联合治疗剖宫产瘢痕憩室(CSD)的效果。方法 2019年1月至2023年6月,选取福建医科大学附属漳州市医院收治的CSD患者110例,按照随机数字表予以分组。对照组(55例)患者采取经阴道憩室修补术治疗,研究组(55例)患者采取宫腹腔镜联合治疗CSD。针对两组手术时间、术中出血量、术后阴道出血时间、住院时间、临床疗效、经期延长时间与术后并发症情况进行比较。结果 两组术中出血量、手术时间对比,结果并无差异性(P> 0.05);研究组术后阴道出血时间、肛门首次排气时间、住院时间均较对照组短(P <0.05)。总有效率评价中,与对照组83.63%相比,研究组96.36%更高(χ2=4.950,P <0.05)。与对照组经期延长时间相比,研究组经期延长时间更短(P <0.05)。术后并发症评价中,两组结果并无差异性(P> 0.05)。结论 宫腹腔镜联合治疗CSD效果确切,可以有效改善患者的经期延长时间,缩短康复进程。

Objective Analyze the effect of hysteroscopy combined with laparoscopy in the treatment of cesarean scar diverticulum(CSD).Methods From January 2019 to June 2023,110 CSD patients admitted to Fujian Medical University Affiliated Zhangzhou Hospital were selected and grouped according to a random number table.The control group(55 cases) received treatment with transvaginal diverticulum repair surgery,while the study group(55 cases) received combined treatment of CSD with hysteroscopy and laparoscopy.Compare the surgical time,intraoperative bleeding volume,postoperative vaginal bleeding time,hospital stay,clinical efficacy,menstrual extension time,and postoperative complications between two groups.Results There was no significant difference in intraoperative bleeding volume and surgical time between the two groups(P>0.05).The study group had shorter postoperative vaginal bleeding time days,time of first anal discharge and hospital stay days compared to the control group days(P<0.05).In the total effective rate evaluation,compared with the control group of 83.63%,the study group of 96.36%was higher(χ2=4.950,P<0.05).Compared with the control group,the study group had a shorter menstrual extension time(P<0.05).Between the two groups,in the evaluation of postoperative complications,there was no difference in the results(P>0.05).Conclusions The combined treatment of hysteroscopy and laparoscopy for CSD has a definite effect,can effectively improve the menstrual extension time of patients,shorten the rehabilitation process.
