LCBDE联合LC治疗胆囊结石合并胆总管结石的疗效分析

Analysis of the Therapeutic Effect of LCBDE Combined with LC in the Treatment of Gallstones with Common Bile Duct Stones

探讨腹腔镜胆总管探查术(LCBDE)联合腹腔镜胆囊切除术(LC)治疗胆囊结石合并胆总管结石患者的临床疗效。选取2021年1月—2023年7月黔东南州人民医院收治的78例胆囊结石合并胆总管结石患者为研究对象,随机将其分为对照组和观察组两组,每组各39例。对照组行内镜十二指肠括约肌切开术(EST)联合LC治疗,观察组行LCBDE联合LC治疗,对比两组患者疗效。结果显示,观察组治疗总有效率高于对照组(P<0.05);观察组手术时间、术中出血量、住院时间等指标均优于对照组,VAS评分低于对照组(P<0.05);术后,观察组Cor、CRP、ACTH、AD等相关指标水平均低于对照组(P<0.05),各项生活质量评分均高于对照组(P<0.05);观察组并发症总发生率低于对照组(P<0.05)。研究发现,LCBDE联合LC治疗胆囊结石合并肝胆总管结石患者疗效显著,其能够减少患者术中出血量,减轻患者术后疼痛,降低患者创伤应激反应水平和并发症发生率,缩短患者的恢复时间,提高患者的生活质量。

Exploring the clinical efficacy of laparoscopic common bile duct exploration(LCBDE)combined with laparoscopic cholecystectomy(LC)in the treatment of patients with gallstones and common bile duct stones.78 patients with gallstones combined with common bile duct stones admitted to Qiandongnan Prefecture People‘s Hospital from January 2021 to July 2023 were selected as the study subjects.They were randomly divided into a control group and an observation group,with 39 patients in each group.The control group underwent endoscopic sphincterotomy(EST)combined with LC treatment,while the observation group underwent LCBDE combined with LC treatment.The efficacy of the two groups of patients was compared.The results showed that the total effective rate of the observation group was higher than that of the control group(P<0.05);The observation group had better surgical time,intraoperative blood loss,hospital stay and other indicators than the control group,and the VAS score was lower than that of the control group(P<0.05);After surgery,the levels of Cor,CRP,ACTH,AD and other related indicators in the observation group were lower than those in the control group(P<0.05),and all quality of life scores were higher than those in the control group(P<0.05);The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Research has found that the combination of LCBDE and LC therapy has significant therapeutic effects on patients with gallstones and common bile duct stones.It can reduce intraoperative bleeding,alleviate postoperative pain,lower levels of traumatic stress response and incidence of complications,shorten recovery time,and improve patients‘quality of life.
