老年慢性阻塞性肺疾病患者健康行为与心理弹性和家庭功能的相关性及护理对策

Health behavior and its correlation with psychological resilience and family function in elderly patients with chronic obstructive pulmonary disease

目的分析老年慢性阻塞性肺疾病(COPD)患者健康行为及与心理弹性、家庭功能的相关性,制定相关的护理对策。方法选取2020年1月至2023年6月江苏省人民医院收治的老年COPD患者作为研究对象,分别以健康促进生活方式量表Ⅱ(HPLPⅡ)、心理弹性量表(CD-RISC)及家庭关怀度指数量表(APGAR)评估老年COPD患者的健康行为、心理弹性及家庭功能;采用Pearson相关系数分析老年COPD患者HPLPⅡ评分与CD-RISC、APGAR评分的相关性;采用多元线性回归分析法分析老年COPD患者健康行为的影响因素。结果研究共纳入96例老年COPD患者,患者HPLPⅡ评分为(79.82±8.43)分,CD-RISC评分为(52.74±6.38)分,APGAR评分为(6.13±1.46)分,均处于中等水平。Pearson相关分析显示,老年COPD患者HPLPⅡ评分与CD-RISC、APGAR评分均呈正相关(P<0.05)。多元线性回归分析显示,CD-RISC评分(β′=0.201)、APGAR评分(β′=0.219)、年龄(β′=0.233)、婚姻状况(β′=0.199)、睡眠质量(β′=0.184)、文化程度(β′=0.212)是老年COPD患者健康行为的影响因素(P<0.05)。结论老年COPD患者健康行为与心理弹性、家庭功能相关,应根据相关影响因素采取针对性护理干预对策,提高患者的健康行为。

Objective To analyze health behaviors and correlations with psychological resilience and family function in elderly patients with chronic obstructive pulmonary disease(COPD),and to designate relevant nursing responses.Methods Elderly patients with COPD admitted to the First Affiliated Hospital with Nanjing Medical University from January 2020 to June 2023 were selected as research objects.Health promoting lifestyle scaleⅡ(HPLPⅡ),Connor-Davidson Resilience Scale(CD-RISC)and Family Adaptation Partnership Gowth and Resolve(APGAR)Index were used to evaluate the health behavior,psychological resilience and family function of elderly COPD patients.Pearson correlation coefficient was used to analyze the correlation of HPLPⅡscore with CD-RISC score and APGAR score in elderly COPD patients.Multiple linear regression analysis was used to analyze the influencing factors of health behavior in elderly patients with COPD.Results A total of 96 elderly patients with COPD were included in the study.HPLPⅡscore was(79.82±8.43),CD-RISC score was(52.74±6.38)and APGAR score was(6.13±1.46),all indicating a medium level.Pearson correlation analysis showed that HPLPⅡscores were positively correlated with CD-RISC and APGAR scores in elderly COPD patients(P<0.05).Multiple linear regression analysis showed that CD-RISC score(β′=0.201),APGAR score(β′=0.219),age(β′=0.233),marital status(β′=0.199),sleep quality(β′=0.184)and educational level(β′=0.212)were the influencing factors of health behavior in elderly patients with COPD(P<0.05).Conclusion The health behavior of elderly patients with COPD is related to psychological resilience and family function.Targeted nursing intervention measures should be taken according to relevant influencing factors to improve the health behavior of patients.
