益气升陷汤加味联合磁刺激治疗子宫脱垂的临床研究

Clinical study of Yiqishengxian decoction with magnetic stimulation in treatment of uterine prolapse

目的探讨益气升陷汤加味联合磁刺激治疗子宫脱垂的临床疗效。方法回顾性分析2019年10月至2021年10月唐山市妇幼保健院妇产科治疗的60例子宫脱垂患者,根据治疗方法分为观察组和对照组,每组30例。给予益气升陷汤加味联合磁刺激治疗的为观察组,给予磁刺激治疗的为对照组,比较两组临床疗效、中医证候积分、盆底肌肌力、盆底功能障碍问卷20(PFDI-20)评分、盆底障碍影响简易问卷7(PFDI-7)评分及1年内的复发率。结果治疗后,观察组临床总有效率(96.7%)高于对照组(70.0%),腰背酸痛、神疲乏力、小腹坠胀、纳呆食少积分低于对照组,差异有统计学意义(P<0.05);观察组治疗后盆底肌肌力Ⅲ级率(66.7%)高于对照组(20.0%),Ⅰ级(0%)、Ⅱ级率(33.3%)低于对照组(13.3%、66.7%),差异有统计学意义(P<0.05);观察组治疗后PFDI-20评分、PFDI-7评分及复发率均低于对照组,差异有统计学意义(P<0.05)。结论益气升陷汤加味联合磁刺激可有效提高子宫脱垂患者盆底肌肌力,改善生活质量,降低复发率。

Objective To investigate the clinical effect of Yiqishengxian decoction combined with magnetic stimulation in the treatment of uterine prolapse.Methods A retrospective analysis was performed on 60 patients with uterine prolapse treated in the Department of Obstetrics and Gynecology of Tangshan Maternal and Child Health Hospital from October 2019 to October 2021, and they were divided into observation group and control group according to treatment methods, with 30 cases in each group. The observation group was given Yiqishengxian decoction combined with magnetic stimulation, and the control group was given magnetic stimulation. The clinical efficacy, TCM syndrome score, pelvic floor muscle strength, Pelvic floor Dysfunction Questionnaire 20(PFDI-20) score, Pelvic floor Dysfunction Simple Questionnaire 7(PFDI-7) score and the recurrence rate within 1 year were compared between the two groups. Results After treatment, the total clinical effective rate of observation group(96.7%) was higher than that of control group(70.0%), and the scores of lumbago and back pain, mental fatigue, low abdominal distension and low eating were lower than those of control group, the differences were statistically significant(P<0.05). The rate of pelvic floor muscle strength grade Ⅲ(66.7%) in the observation group was higher than that in the control group(20.0%), the rate of grade Ⅰ(0%) and grade Ⅱ(33.3%) were lower than those in the control group(13.3%, 66.7%), and the differences were statistically significant(P<0.05). PFDI-20 score, PFDI-7 score and recurrence rate in the observation group were lower than those in the control group after treatment, and the differences were statistically significant(P<0.05).Conclusion Yiqishengxian decoction combined with magnetic stimulation can effectively improve the pelvic floor muscle strength, improve the quality of life and reduce the recurrence rate of patients with uterine prolapse.
