加味桃红四物汤联合冰硝散外敷促进单髁置换术后早期康复作用

Clinical Effect of Modified Taohong Siwu Decoction Combined with External Binxiao Powder on Early Rehabilitation after Unicompartmental knee Arthroplasty

目的:观察加味桃红四物汤联合冰硝散外敷在单髁置换术(UKA)后早期康复中的临床疗效。方法:选取2023年1月-2023年12月在安徽中医药大学附属六安市中医院因膝骨性关节炎住院拟首次行UKA的60例患者为研究对象,按随机数字表法分为观察组和对照组各30例。对照组予以常规治疗,观察组在对照组的基础上术后第1天予以口服加味桃红四物汤联合冰硝散外敷,持续治疗两周。比较2组患者不同时间点的视觉模拟(VAS)评分、膝关节肿胀值、关节活动度(ROM)、美国特种外科医院(HSS)评分和血清炎症因子(IL-6、TNF-α)水平。结果:术后1周和2周观察组的VAS评分、膝关节肿胀值和血清炎症因子(IL-6、TNF-α)水平低于对照组(均P<0.05),膝关节ROM和HSS评分高于对照组(均P<0.05),2组均未出现明显的不良反应。结论:加味桃红四物汤联合冰硝散外敷能显著缓解UKA后肿痛、改善关节活动度和抑制炎症反应而有利于患者术后早期康复,疗效确切,值得临床推广应用。

Objective:To observe the clinical efficacy of modified Taohong Siwu Decoction combined with Bingxiaosan external application in early rehabilitation after unicompartmental knee arthroplasty(UKA).Methods:A total of 60 patients who were hospitalized for UKA for the first time in Lu’an Traditional Chinese Medicine Hospital affiliated to Anhui University of Traditional Chinese Medicine due to knee osteoarthritis from January 2023 to December 2023 were selected as the research subjects and were divided into an observation group and a control group according to the random number table method,with 30 cases in each group.The control group was given conventional treatment,and the observation group was given oral modified Taohong Siwu Decoction combined with Bingxiaosan external application on the first day after surgery on the basis of the control group,and the treatment lasted for two weeks.The visual analog(VAS)score,knee swelling value,joint range of motion(ROM),American Hospital for Special Surgery(HSS)score and serum inflammatory factors(IL-6,TNF-α)levels of patients in the two groups at different time points were compared.Results:At 1 week and 2 weeks after surgery,the VAS score,knee swelling value and serum inflammatory factors(IL-6,TNF-α)levels of the observation group were lower than those of the control group(P<0.05),and the knee ROM and HSS scores were higher than those of the control group(P<0.05).No obvious adverse reactions occurred in the two groups.Conclusion:Modified Taohong Siwu Decoction combined with Bingxiao powder external application can significantly relieve swelling and pain after UKA,improve joint mobility and inhibit inflammatory response,which is beneficial to the early rehabilitation of patients after surgery.It has a definite effect and is worthy of clinical promotion and application.
