摘要
目的分析柴胡桂枝干姜汤证患者的红外热成像特征。方法选择2021年4月至2023年7月在广州中医药大学附属中山中医院内分泌科李乐愚门诊就诊的32例柴胡桂枝干姜汤证患者作为试验组,同期选取32名健康体检者作为对照组,对两组受试者的红外热成像结果进行特定部位的温度区分,并进行相关性分析。对性别、年龄及头面、前躯干、左肋/左乳、右肋/右乳、任脉、督脉温度信息进行单因素分析和Kmeans聚类分析,以是否符合柴胡桂枝干姜汤证作为因变量,进行Logistic回归分析。结果两组受试者年龄及左肋/左乳、右肋/右乳、督脉温度比较差异无统计学意义(P>0.05)。试验组头面、前躯干、任脉温度均高于对照组[33.8(33.3,34.2)℃比32.2(31.0,33.0)℃、(33.11±1.26)℃比(32.47±1.01)℃、(33.23±1.14)℃比(32.21±1.29)℃](P<0.05或P<0.01)。Kmeans聚类分析结果显示,左肋/左乳、前驱干、督脉、右肋/右乳温度在红外成像领域的贡献度较高,而年龄的影响力相对较小。多因素Logistic回归分析结果显示,头面、任脉温度会对柴胡桂枝干姜汤证证型的判断产生显著性影响(OR=5.861,95%CI 2.197~15.637;OR=7.878,95%CI 1.046~59.315)(P<0.05或P<0.01)。模型公式:ln(P/1-P)=70.504+1.768×头面温度+2.064×任脉温度。结论将红外热成像技术与人工智能算法相结合,对于分析复杂的中医方证具有重要意义。在治疗前进行成像特征分析,可为胡桂枝干姜汤证的临床诊断和治疗提供有价值的参考。
Objective To analyze the characteristics of infrared thermal imaging of patients with Chaihu Guizhi Ganjiang decoction syndrome.Methods 32 patients with Chaihu Guizhi Ganjiang decoction syndrome from LI Leyu Outpatient Department of Endocrinology,Zhongshan Hospital of Traditional Chinese Medicine(TCM)from Apr.2021 to Jul.2023,Guangzhou University of Traditional Chinese Medicine were included as a test group,and 32 healthy physical examination subjects during the same period were included as a control group.The infrared thermal imaging results of the two groups were differentiated by temperature of specific parts,and correlation analysis was conducted.Univariate analysis and Kmeans cluster analysis were performed on the temperature information of age,gender,head,front torso,left rib/breast,right rib/breast,Ren channel and Du channel,etc.Logistic regression analysis was carried out using whether it conforms to Chaihu Guizhi Ganjiang decoction syndrome as the dependent variable.Results There was no significant temperature difference by age and left rib/breast,right rib/breast,and Du channel between the two groups(P>0.05).The temperature of head,front torso and Ren channel of the test group were higher than those of the control group[(33.8(33.3,34.2)℃vs 32.2(31.0,33.0)℃,(33.11±1.26)℃vs(32.47±1.01)℃,(33.23±1.14)℃vs(32.21±1.29)℃](P<0.05 or P<0.01).Kmeans cluster analysis showed that the left rib/breast,front torso,Du channel,and right rib/breast temperature had high contributions in the infrared imaging field,while age had relatively less influence.The results of multivariate Logistic regression analysis showed that the temperature of head surface and Ren channel had a significant impact on the judgment of Chaihu Guizhi Ganjiang decoction syndrome(OR=5.861,95%CI 2.197-15.637;OR=7.878,95%CI 1.046-59.315)(P<0.05 or P<0.01).Model formula:ln(P/1-P)=70.504+1.768×head surface temperature+2.064×Ren channel temperature.Conclusion The combination of infrared thermal imaging technology and artificial intelligence algorithm is of great significance for analyzing complex TCM formula and syndrome.Imaging feature analysis before treatment can provide valuable reference for the clinical diagnosis and treatment.
