慢性乙型病毒性肝炎患者心理困扰水平及其相关影响因素分析

Analysis of psychological distress level and its related influencing factors in patients with chronic viral hepatitis B

目的探讨慢性乙型病毒性肝炎(CHB)患者心理困扰水平的影响因素,以指导临床干预措施的拟定,减轻CHB患者的心理困扰水平。方法选取2020年9月至2023年9月萍乡市第二人民医院收治的80例慢性乙型病毒性肝炎患者作为研究对象,评估所有患者的心理困扰水平[心理困扰量表(Kessler-10)],设计基线资料调查表,详细统计所有患者的基线资料,比较不同资料特征CHB患者的Kessler-10评分,重点分析CHB患者心理困扰水平的影响因素。结果经评估,80例CHB患者的Kessler-10评分为(23.45±3.34)分;不同病程、心理弹性、社会支持及合并基础疾病CHB患者的Kessler-10评分比较,差异有统计学意义(P<0.05);不同性别、年龄、抗病毒治疗、家庭月人均收入、家族史、文化水平及婚姻状况CHB患者的Kessler-10评分比较,差异无统计学意义(P>0.05);经多元线性回归分析发现,病程长、心理弹性低下、合并其他慢性病及社会支持低均是CHB患者心理困扰水平的影响因素(P<0.05)。结论CHB患者心理困扰呈中等水平,CHB患者心理困扰水平的影响因素为病程长、心理弹性低下、合并其他慢性病及社会支持低。

Objective To explore the influencing factors of psychological distress levels in patients with chronic hepatitis B(CHB)to guide the development of clinical intervention measures and alleviate the psychological distress levels of CHB patients.Methods A total of eighty patients with CHB admitted to the Second People's Hospital of Pingxiang City from September 2020 to September 2023 were selected as the study objects.The level of psychological distress(psychological distress scale[Kessler-10])of all patients was assessed,the baseline data questionnaire was designed,the baseline data of all patients in detail was counted,the Kessler-10 scores of CHB patients with different data characteristics were compared,and the analysis of the CHB patients'psychological factors influencing the level of distress in CHB patients was focused on.Results The Kessler-10 score of 80 CHB patients was assessed to be(23.45±3.34),and the Kessler-10 scores of CHB patients with different disease duration,psychological resilience,social support and co-morbidities were compared,and the differences were statistically significant(P<0.05).The Kessler-10 scores of CHB patients with different genders,ages,antiviral treatments,per capita monthly household incomes,family histories,cultural level and marital status of CHB patients were compared,the differences were not statistically significant(P>0.05).The multiple linear regression analysis found that the long duration of the disease,low psychological resilience,comorbidities with other chronic diseases and low social support were all factors influencing the level of psychological distress in CHB patients(P<0.05).Conclusion The psychological distress of CHB patients is at a moderate level,and the influencing factors of their psychological distress level are long disease course,low psychological resilience,comorbidities with other chronic diseases,and low social support.
