正畸联合口腔修复治疗先天性缺牙患者的效果及对龈沟液相关指标咀嚼功能的影响

Effect of orthodontics combined with prosthodontics on the treatment of patients with congenital missing teeth and its influence on GCF related indicators and masticatory function

目的 研究正畸联合口腔修复对先天性缺牙的治疗效果及对龈沟液相关指标、咀嚼功能的影响。方法 依据患者个人修复意愿将2020年4月至2022年9月我院收治的92例先天性缺牙患者分为对照组、观察组,对照组45例采用口腔修复方式,观察组47例在此基础上联合正畸治疗。比较2组修复效果、不良事件及修复前后牙齿功能、牙周指标[龈沟出血指数(SBI)、菌斑指数(PLI)、牙齿松动度(TM)]、龈沟液相关指标[白细胞介素-6(IL-6)、IL-33、基质金属蛋白酶8(MMP-8)]。结果 观察组修复总有效率96%较对照组80%高(P<0.05);修复后观察组咀嚼功能、牙齿美观度、语言功能评分较对照组升高(P<0.05);修复后观察组SBI、PLI、TM评分低于对照组(P<0.05);修复后观察组IL-6、IL-33、MMP-8较对照组降低;观察组不良事件总发生率6%较对照组22%降低(P<0.05)。结论 口腔修复联合正畸能改善先天性缺牙患者牙周指标与牙齿功能,减轻龈沟液相关炎性因子水平,降低不良事件发生率,提高修复效果。

Objective To study the therapeutic efect of orthodontics combined with oral prosthetics on congenitally missing teeth and its effect on gingival sulcus fluid related indexes and masticatory function.Methods According to the patients'individual restorative wishesr 92 congenitally edentulous patients admitted to our hospital from April 2020 to September 2022 were divided into a control group and an observation group,45 cases in the control group were treated with oral restorations,and 47 cases in the observation group were combined with orthodontic treatment on this basis.The control group was treated with oral repair,and the observation group was treated with orthodontic combined oral repair.The repair effect,adverse events,dental function before and after repair,periodontal indexes[gingival bleeding index(SBI),plaque index(PLI),tooth mobility(TM)],gingival fluid related indexes[interleukin-6(IL-6),IL-33,matrix metalloproteinase 8(MMP-8)]were compared between the two groups.Results The total effective rate of observation group was 96%,which was higher than that of control group(80%)(P<0.05).After restoration,the scores of masticatory function,dental aesthetics and language function in the observation group were higher than those in the control group(P<0.05).After repair,the scores of SBI,PLI and TM in observation group were lower than those in control group(P<0.05).The total incidence of adverse events was 6%,which was lower in the observation group compared to 22%in the control group(P<0.05).Conclusion Restorative dentistry combined with orthodontics improves periodontal indicators and tooth function,attenuates gingival sulcus fluid-associated inflammatory factor levels,reduces the incidence of adverse events,and improves restorative outcomes in patients with congenitally missing teeth.
