摘要
目的利用图论研究年龄相关性听力损失(ARHL)患者脑结构网络拓扑属性变化,探讨ARHL患者脑结构网络改变与认知的关系。资料与方法对2022年10月—2023年10月安顺市人民医院31例ARHL患者和27例健康对照者行扩散张量成像扫描及认知量表测试,基于图论分析脑网络拓扑属性改变,全局属性包括小世界属性、聚类系数、特征路径长度、全局效率、局部效率、网络同配性;局部属性包括节点集群系数、节点最短路径、节点效率、节点局部效率、节点中心度、节点介数中心度;并探讨异常脑网络拓扑属性与认知的相关性。结果与对照组相比,ARHL组特征路径长度增加,全局效率、局部效率减小(t=3.08、-3.15、-3.30,P均<0.01);右眶部额下回节点最短路径增加,左背外侧额上回、双侧眶部额下回、左内侧额上回节点效率减低,左背外侧额上回、左顶上回、左楔前叶节点中心度降低(t=-4.74~5.19,P均<0.001)。ARHL组蒙特利尔认知评估量表评分与特征路径长度呈负相关(r=-0.390),与全局效率(r=0.390)、左楔前叶节点中心度(r=0.466)呈正相关(P均<0.05);ARHL组简易精神状态检查评分与特征路径长度(r=-0.473)呈负相关,与全局效率(r=0.473)、局部效率(r=0.408)、左楔前叶中心度(r=0.363)呈正相关(P均<0.05)。结论ARHL患者脑网络拓扑属性存在异常,异常脑区主要分布于额叶和顶叶,其中左楔前叶的异常改变可能在认知功能障碍的形成中起关键作用。
Purpose Tostudythe changes of topological properties of brain structure network in age-related hearingloss(ARHL)by using graph theory,and to explore the relationship between the changes of brain structure network and cognition.Materials and Methods A total of 31 patients with ARHL and 27 controls from October 2022 to October 2023 in People’s Hospital of Anshun City were recruited to undergo a diffusion tensor imaging scan and cognitive tests.Based on graph theory,this paper analyzes the changes of topological attributes of brain networks.The global attributes included small-world attribute,clustering coefficient,characteristic path length,global efficiency,local efficiency and network compatibility.The local attributes included nodal clustering coefficient,nodal shortest length,node efficiency,nodal local efficiency,nodal degree centrality and nodal betweenness centrality.And the correlation between abnormal brain network topology and cognition were explored.Results Compared with the control group,the characteristic path length in ARHL group significantly increased,and the global efficiency and local efficiency significantly decreased(t=3.08,-3.15,-3.30;all P<0.01);the nodal shortest length of the right orbital inferior frontal gyrus increases,while the node efficiency of the left dorsolateral superior frontal gyrus,bilateral orbital inferior frontal gyrus and left medial superior frontal gyrus decreases,and the nodal degree centrality of the left dorsolateral superior frontal gyrus,left superior parietal gyrus and left precuneus decreases(t=-4.74-5.19;all P<0.001).The Montreal cognitive assessment score in ARHL group was negatively correlated with the characteristic path length(r=-0.390,P=0.030),and positively correlated with the global efficiency(r=0.390,P=0.030)and the nodal degree centrality value of left precuneus(r=0.466,P=0.008).The mini-mental state examination score was negatively correlated with the characteristic path length(r=-0.473,P=0.007),and with the global efficiency(r=0.473,P=0.007),local efficiency(r=0.408,P=0.023),and the nodal degree centrality value of left precuneus(r=0.363,P=0.363).Conclusion There are abnormalities in the topological properties of the brain network in ARHL,and the abnormal brain regions are mainly distributed in the prefrontal and parietal lobe,among which the abnormal changes in the left precuneus may play a key role in cognitive dysfunction.
