摘要
目的:研究CT能谱成像技术在甲状腺结节鉴别诊断方面的应用价值。方法:对96例甲状腺结节患者(收治时间:2021年1月至2023年6月)的临床资料进行回顾性分析。根据病理检查结果将患者分为良性组(40例)和恶性组(56例),并记录比较不同期相(平扫期、动脉期和静脉期)的碘浓度、有效原子序数以及能谱衰减曲线斜率。结果:组间碘浓度对比可见,恶性组的平扫期(6.53±0.62)mg/mL、动脉期(8.99±1.93)mg/mL和静脉期(16.02±3.02)mg/mL与良性组对比均形成显著差异(P<0.05);组间有效原子序数对比可见,恶性组数据与良性组对比无显著差异(P>0.05);组间能谱衰减曲线斜率对比可见,恶性组的平扫期(0.66±0.20)、动脉期(3.02±0.22)和静脉期(3.80±0.38)与良性组对比均形成显著差异(P<0.05)。结论:CT能谱成像技术通过对比不同时间点的碘浓度和分析能谱衰减曲线的斜率,能够精准判断甲状腺结节的良性和恶性特征,提高了诊断的准确性。
Objective:To study the application value of CT energy spectrum imaging technology in the dfferential diagnosis of thyroid nodules.Methods:A retrospective analysis was conducted on the clinical data of 96 patients with thyroid nodules(admitted from January 2021 to June 2023).According to the pathological examination results,the patients were divided into a benign group(40 cases)and a malignant group(56 cases),and the iodine concentration,effective atomic number,and slope of the energy spectrum attenuation curve were recorded and compared during different periods(plain scan,arterial phase,and venous phase).Result:The comparison of iodine concentration between groups showed that there were significant differences(P<0.05)between the malignant group and the benign group in the plain scan phase(6.53±0.62)mg/mL,arterial phase(8.99±1.93)mg/mL,and venous phase(16.02±3.02)mg/mL(P<0.05).The comparison of effective atomic numbers between groups showed no significant difference(P>0.05)between the malignant group and the benign group;The comparison of the slope of the energy spectrum atenuation curve between groups shows that there are significant differences(P<0.05)between the malignant group and the benign group in the plain scan phase(0.66±0.20)mg/mL,arterial phase(3.02±0.22)mg/mL,and venous phase(3.80±0.38)mg/mL.Conclusion:CT energy spectrum imaging technology can accurately determine the benign and malignant characteristics of thyroid nodules by comparing iodine concentrations at different time points and analyzing the slope of the energy spectrum attenuation curve,thereby improving the accuracy of diagnosis.
