动态颅内压监护下双侧脑室外引流结合术后血肿腔内尿激酶注入治疗脑室内出血的疗效观察

摘要 目的研究动态颅内压监护下双侧脑室外引流结合术后血肿腔内尿激酶注入治疗脑室出血的疗效观察.方法选取我院2018年1月-2023年1月52例脑室内出血患者,其中23例行双侧脑室外引流结合动态颅内压监护,术后血肿腔内尿激酶注入,29例行单侧脑室外引流术+颅内压探头置入术.观察两组患者术后动态颅内压监护值、血清NSE指标及预后情况.结果两组患者术后动态颅内压监护值、血清NSE指标及预后组间比较,有显著差异(P<0.05).结论动态颅内压监护下双侧脑室外引流结合术后血肿腔内尿激酶注入较传统的单侧脑室外引流术有明显的疗效,对于降低患者术后死亡率、致残率及改善生活质量有确切的临床意义. Objective To investigate the therapeutic effect of bilateral ventricular external drainage under dynamic intracranial pressure monitoring combined with urokinase injection in postoperative hematoma cavity on ventricular hemorrhage.Methods From January 2018 to January 2023,52 patients with intraventricular hemorrhage were selected from our hospital,among which 23 cases were treated with bilateral ventricular external drainage combined with dynamic intracranial pressure monitoring,and postoperative urokinase was injected into the hematoma cavity.29 cases were performed with unilateral ventricular external drainage and intracranial pressure probe.The monitoring value of postoperative dynamic intracranial pressure,blood NSE index and prognosis of the two groups were observed.Results There were significant differences in postoperative monitoring values of dynamic intracranial pressure,blood NSE and prognosis between the two groups(P<0.05).Conclusion Bilateral extracentricular drainage under dynamic intracranial pressure monitoring combined with urokinase injection in postoperative hematoma cavity has obvious curative effect compared with traditional unilateral extracentricular drainage,which has definite clinical significance in reducing postoperative mortality rate,disability rate and improving quality of life.
