摘要
在中医药领域,中药饮片的准确调剂对于保障患者用药安全和治疗效果至关重要.然而,复核环节中频繁发现的调剂差错不仅增加了工作压力,还严重影响了中药的调剂质量.此研究通过分析北京中医药大学房山医院中药房2023年3月-4月中药饮片调剂过程中复核环节出现的内部差错,并基于此制定并实施有效的风险防控策略,随后2023年6月-7月再次分析中药饮片调剂过程中复核环节出现的内部差错,以评估防控策略的效果.研究结果表明,实施风险防控策略后,调剂差错率显著降低,具有统计学意义(P<0.05),差错类型包括未审方、漏配、规格错配、药物错配、多配等,均有效得到控制.研究结论强调,通过对复核环节差错的系统分析和针对性的防控措施实施,可以有效减少中药饮片调剂过程中的差错,确保患者用药安全,提高医院药房的服务质量.此研究为医院药房中药饮片调剂过程中的质量改进提供了实用的参考策略.
In the field of Traditional Chinese Medicine,accurate adjustment of Traditional Chinese Medicine decoction pieces is crucial for ensuring patient medication safety and treatment effectiveness.However,frequent dispensing errors discovered during the review process not only increase work pressure,but also seriously affect the quality of dispensing Traditional Chinese Medicine.This study analyzed the internal errors that occurred during the review process of Traditional Chinese Medicine decoction dispensing at Fangshan Hospital of Beijing University of Chinese Medicine from March to April 2023,and based on this,developed and implemented effective risk prevention and control strategies.Subsequently,from June to July 2023,the internal errors that occurred during the review process of Traditional Chinese Medicine decoction dispensing were analyzed again to evaluate the effectiveness of the prevention and control strategies.The research results showed that after the implementation of risk prevention and control strategies,the dispensing error rate was significantly reduced,with statistical significance(P<0.05).The error types,including unapproved formula,missed dispensing,specification mismatch,drug mismatch,and multiple dispensing,were effectively controlled.The research conclusion emphasizes that through systematic analysis of errors in the review process and targeted prevention and control measures,errors in the dispensing process of Traditional Chinese Medicine can be effectively reduced,ensuring patient medication safety and improving the service quality of hospital pharmacies.This study provides a practical reference strategy for quality improvement in the dispensing process of traditional Chinese medicine decoction pieces in hospital pharmacies.
