针刀联合桂枝汤治疗屈指肌腱狭窄性腱鞘炎的临床研究

Clinical study on the treatment of stenosing tenosynovitis of the flexor digitorum tendon with needle knife combined with Guizhi decoction

目的分析针刀联合桂枝汤治疗屈指肌腱狭窄性腱鞘炎的临床效果.方法选取2023年1月-2023年12月在泰顺县中医院收治的80例屈指肌腱狭窄性腱鞘炎患者,随机分为对照组和观察组各40例.对照组应用局部封闭治疗,观察组应用针刀联合桂枝汤治疗.对比两组患者治疗效果、治疗前后疼痛程度[视觉模拟评分法(VAS)]、关节活动度(ROM)及昆内尔分级(Quinnell),并统计两组患者不良反应发生率.结果观察组患者治疗总有效率为90%,高于对照组的70%,P<0.05;治疗1周后,观察组VAS评分低于对照组,且ROM高于对照组,P<0.05;观察组Quinnell分级优于对照组,P<0.05;观察组患者不良反应发生率为10%,远低于对照组的30%,P<0.05.结论针刀联合桂枝汤治疗相较于局部封闭治疗对屈指肌腱狭窄性腱鞘炎患者临床效果更好,可明显改善患者疼痛程度及关节活动度,且安全性较高.

Objective To analyze the clinical effect of needle knife combined with Guizhi decoction in the treatment of stenosing tenosynovitis of the flexor digitorum tendon.Methods 80 patients with stenosing tenosynovitis of the flexor digitorum tendon admitted to Taishun County Traditional Chinese Medicine Hospital from January 2023 to December 2023 were chosen as the study subjects.They were randomly split into a control group and an observation group with 40 cases.The control group patients were treated with local blockade,while the observation group patients were treated with needle knife combined with Guizhi decoction.Compare the treatment efficacy,pain level before and after treatment[Visual Analog Scale(VAS)],ROM and Quinnell scale between two groups of patients,and calculate the incidence of adverse reactions in the two groups of patients.Results The effective rate of treatment in the observation group was 90%,higher than 70%in the control group,with P<0.05;the VAS score of the observation group was lower than that of the control group,and the ROM was higher than that of the control group,P<0.05;the Quinnell grading of the observation group was better than the control group,P<0.05;the incidence of adverse reactions in the observation group was 10%,much less than the 30%in the control group,with P<0.05.Conclusion The combination of needle knife and Guizhi decoction has a better clinical effect on patients with stenosing tenosynovitis of the flexor digitorum tendon compared to local closure treatment.It can clearly improve the pain level and joint mobility of patients and has higher safety.
