活血化瘀汤联合常规西医疗法治疗气虚血瘀证脑梗死的效果及对血流动力学、神经功能的影响

Therapeutic effect of Huoxue Huayu decoction combined with conventional western medicine on cerebral infarction of Qi deficiency and blood stasis syndrome and its influence on hemodynamics and neurological function

目的探讨气虚血瘀证脑梗死(CI)患者行活血化瘀汤联合常规西医疗法治疗的效果及对血流动力学及神经功能的影响。方法队列研究。抽取漯河市中医院2021年1月至2022年12月收治的气虚血瘀证CI患者96例,按随机数字表法分为西医组(48例)和中西联合组(48例)。西医组采用常规西医治疗,中西联合组在西医组基础上行活血化瘀汤治疗。比较两组疗效、炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、血流动力学[血浆黏度(PV)、全血高切黏度(HWBV)、全血低切黏度(LWBV)]水平、美国国立卫生研究院卒中量表(NIHSS)评分及Barthel指数。结果中西联合组总有效率(93.75%,45/48)较西医组(77.08%,37/48)高(P<0.05)。治疗后,与西医组比较,中西联合组CRP、IL-6、TNF-α、PV、HWBV、LWBV水平较低,NIHSS评分较低、Barthel指数较高(P<0.05)。结论活血化瘀汤联合西医治疗气虚血瘀证CI效果显著,能减轻机体炎症,改善患者血流动力学和神经功能,提升其日常生活能力。

Objective To investigate the effect of Huoxue Huayu decoction combined with conventional western medicine on patients with cerebral infarction(CI)of Qi deficiency and blood stasis syndrome and its influence on hemodynamics and nerve function.Methods:Ninety-six patients with CI of Qi deficiency and blood stasis syndrome treated in Luohe Hospital of Tranditional Chinese Medicine from January 2021 to December 2022 were selected for the cohort study.And they were divided into a western medicine group(48 cases)and an integrated Chinese and western medicine group(48 cases)according to random number table method.The western medicine group was treated with conventional western medicine,and the integrated Chinese and western medicine group was treated with Huoxue Huayu decoction based on the treatment of the western medicine group.The curative effect,inflammatory factors,including C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α),levels of hemodynamics,including plasma viscosity(PV),high-shear whole blood viscosity(HWBV)and low-shear whole blood viscosity(LWBV),American National Institutes of Health stroke scale(NIHSS)score and Barthel index of the two groups were compared.Results The total effective rate of the integrated Chinese and western medicine group(93.75%,45/48)was higher than that of the western medicine group(77.08%,37/48),P<0.05.After treatment,the levels of CRP,IL-6,TNF-α,PV,HWBV and LWBV in the integrated Chinese and western medicine group were lower than those in the western medicine group,while the integrated Chinese and western medicine group had lower NIHSS score and higher Barthel index compared with the western medicine group(P<0.05).Conclusions Huoxue Huayu decoction combined with western medicine has remarkable effects in the treatment of CI of Qi deficiency and blood stasis syndrome.It can reduce the body inflammation,alleviate the hemodynamics and nerve function of patients,and improve their activities of daily living.
