摘要
高值医用耗材是指直接作用于人体、对安全性有严格要求、临床使用量大、价格相对较高、经济负担重的医用耗材。人工晶体作为植入人体的高值医用耗材,需要更严格的管控。随着医院眼科手术量的增加和日间手术平均住院时长的缩短,人工验收、计费的人工晶体保障模式已不能满足临床的需求。该研究通过收集相关数据,利用鱼骨图进行原因分析,利用FOCUS-PDCA法,从成立人工晶体保障小组、改进人工晶体保障流程、优化物资管理系统、强化供应商管理4个方面实施改进措施,并根据改进前后的量化标准评价改进效果。结果显示,与实施FOCUS-PDCA管理前相比,实施FOCUS-PDCA管理后的人工晶体验收时间及计费完成时间均缩短,差异有统计学意义(P<0.05)。因此,运用质量管理工具FOCUS-PDCA改进人工晶体的保障环节,可提高人工晶体的保障质量和保障效率,对保障患者安全和提升医疗质量具有重要意义。
High-value medical consumables refer to medical consumables that directly act on the human body,have strict safety requirements,are widely used in clinical practice,have relatively high prices,and bear heavy costs for the public.As high-value medical consumables implanted in the human body,artificial lenses require stricter control.With the increase in the number of ophthalmic surgeries in hospitals and the shortening of hospitalization days for daytime patients,the existing guarantee mode of manually accepted and billed intraocular lenses can no longer meet the clinical needs.This study collected relevant data,analyzed the reasons using fishbone diagrams,and used the FOCUS-PDCA method to implement improvements in four aspects:establishing an artificial lens support team,improving the artificial lens support process,optimizing the material management system,and strengthening supplier management.The improvement effect was evaluated based on quantitative standards before and after the improvement.The results showed that the acceptance time of human crystals and billing completion time were shortened after the implementation of FOCUS-PDCA management compared with those before the implementation of FOCUS-PDCA management,and the difference was statistically significant(P<0.05).Therefore,using quality management tools to improve the assurance process of artificial lenses can improve the quality and efficiency of intraocular lens assurance,which is of great significance in safeguarding patient safety and improving medical quality.
