中西医结合治疗老年重症肺炎合并呼吸衰竭的临床效果分析

Clinical Effect of Integrated Traditional Chinese and Western Medicine on Elderly Patients with Severe Pneumonia Complicated with Respiratory Failure

目的:分析中西医结合治疗老年重症肺炎合并呼吸衰竭的临床效果。方法:选取2022年10月—2024年1月玉溪市中医医院收治的66例老年重症肺炎合并呼吸衰竭患者作为研究对象,采用随机数字表法分为对照组和观察组,每组33例。对照组给予常规西药治疗,观察组在对照组基础上给予中药治疗。比较两组中医证候评分、血气分析指标、炎性因子水平及不良反应发生情况。结果:治疗后,两组主证、次证评分均低于治疗前,且观察组比对照组低(P<0.05)。治疗后,两组动脉二氧化碳分压均低于治疗前,且观察组比对照组低,两组动脉血氧分压、动脉血氧饱和度均高于治疗前,且观察组较对照组高(P<0.05)。治疗后,两组白细胞介素-6、C反应蛋白、降钙素原水平均低于治疗前,且观察组较对照组低(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P=0.492)。结论:中西医结合治疗老年重症肺炎合并呼吸衰竭的临床效果显著,能够缓解患者的症状,改善其肺功能,抑制炎性反应。

Objective:To analyze the clinical effect of integrated traditional Chinese and western medicine on the elderly with severe pneumonia complicated with respiratory failure.Methods:Sixty-six elderly patients with severe pneumonia combined with respiratory failure admitted to Yuxi Hospital of Traditional Chinese Medicine from October 2022 to January 2024 were selected as the study objects,and were divided into control group and observation group by random number table method,with thirty-three cases in each group.The control group was given conventional western medicine treatment,and the observation group was given traditional Chinese medicine treatment on the basis of the control group.TCM syndrome score,blood gas analysis index,inflammatory factor level and occurrence of adverse reactions were compared between the two groups.Results:After treatment,the scores of primary syndrome and secondary syndrome in both groups were lower than before treatment,and the observation group was lower than the control group(P<0.05);after treatment,the arterial partial pressure of carbon dioxide in both groups was lower than before treatment,and the observation group was lower than the control group,the partial pressure of arterial blood oxygen and saturation of arterial blood oxygen in both groups were higher than before treatment,and the observation group was higher than the control group(P<0.05);after treatment,the average levels of interleukin-6,C-reactive protein and calcitonin in two groups were lower than before treatment,and the observation group was lower than the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P=0.492).Conclusion:The combination of Chinese and western medicine in the treatment of severe pneumonia complicated with respiratory failure in the elderly has remarkable clinical effect,which can alleviate the symptoms of patients,improve their lung function and inhibit inflammatory response.
