摘要
目的:分析经子宫后路分离膀胱反折联合宫颈提拉缝合术在凶险性前置胎盘孕妇中的应用效果。方法:选取2021年6月—2024年6月邵阳学院附属第一医院收治的凶险性前置胎盘孕妇60例作为研究对象,随机分为对照组和研究组,各30例。对照组行宫颈环扎术,研究组行经子宫后路分离膀胱反折联合宫颈提拉缝合术。比较两组术中出血量、术后24 h出血量、输血量、膀胱损伤率、子宫切除率。结果:研究组术中出血量、产后24 h出血量、输血量均少于对照组,差异有统计学意义(P<0.001)。研究组膀胱损伤率低于对照组,差异有统计学意义(P=0.042)。两组均无子宫切除。结论:经子宫后路分离膀胱反折联合宫颈提拉缝合术在凶险性前置胎盘孕妇中的应用效果显著,可减少患者出血量及输血量,减轻膀胱损伤,值得临床应用。
Objective:To analyze the effect of posterior transuterine separation of bladder reflexes combined with cervical lift suture in pregnant women with dangerous placenta previa.Methods:Sixty pregnant women with dangerous placenta previa admitted to the First Affiliated Hospital of Shaoyang University from June 2021 to June 2024 were selected as the study objects and randomly divided into control group and study group,with thirty cases in each group.The control group underwent cervix cerigation,and the study group underwent posterior uterine separation of bladder reflexion combined with cervical lift suture.The intraoperative blood loss,24 h postoperative blood loss,blood transfusion,bladder injury rate and hysterectomy rate were compared between the two groups.Results:The amount of intraoperative blood loss,24 h postpartum blood loss and blood transfusion in the study group was lower than that in the control group,and the difference was statistically significant(P<0.001);the rate of bladder injury in the study group was lower than that in the control group,the difference was statistically significant(P=0.042).Conclusion:Posterior uterine separation of bladder reflexes combined with cervical lift suture is effective in pregnant women with dangerous placenta previa,which can reduce the amount of blood loss and blood transfusion and alleviate bladder injury,and is worthy of clinical application.
