摘要
目的:分析右美托咪定(Dex)、瑞芬太尼复合及丙泊酚、瑞芬太尼复合在小儿麻醉维持喉罩通气中的应用效果。方法:选取2022年7月—2023年6月于徐州市贾汪区人民医院行手术治疗的患儿100例作为研究对象,经抽签法均分为对照组(n=50例,丙泊酚、瑞芬太尼复合麻醉)、观察组(n=50例,Dex、瑞芬太尼复合麻醉),对两组血流动力学、不良反应发生率进行比较。结果:入室时,两组血流动力学指标[血氧饱和度(SPO_(2))、呼气末二氧化碳分压(PCO_(2))、平均动脉压、心率]无显著差异(P>0.05)。麻醉诱导后、置入喉罩时、手术结束时、取出喉罩时、取出喉罩15 min时,对照组SPO_(2)下降,且低于观察组(P<0.05);两组PCO_(2)、平均动脉压、心率均下降,但观察组高于对照组(P<0.05)。观察组不良反应发生率为4.00%、低于对照组(P<0.05)。结论:丙泊酚、瑞芬太尼复合麻醉与Dex、瑞芬太尼复合麻醉均可在小儿麻醉维持喉罩通气中发挥理想作用,但后者更能保持患儿血流动力学稳定性,且麻醉安全性更好。
Objective:Analysis of the effects of dexmedetomidine(Dex),remifentanil combination and propofol,remifentanil combination in pediatric anesthesia to maintain laryngeal mask ventilation.Methods:100 cases of children who underwent surgical treatment in Jiawang District People's Hospital of Xuzhou City from July 2022 to June 2023 were selected as the study subjects,and there were 2 groups equally grouped by the lottery method,which were the control group(n=50 cases,propofol,remifentanil compound anesthesia),and the observation group(n=50 cases,Dex,remifentanil compound anesthesia),and the hemodynamics,adverse effects and the incidence of adverse reactions were compared between the two groups.Comparison of hemodynamics and incidence of adverse reactions between the two groups was made.Results:At the time of admission,there was no significant difference in hemodynamic indexes[oxygen saturation(SPO_(2)),partial pressure of carbon dioxide at end-expiration(PCO_(2)),mean arterial pressure,heart rate]between the two groups(P>0.05).After the induction of anesthesia,when the laryngeal mask was placed,at the end of surgery,when the laryngeal mask was removed,and when the laryngeal mask was removed for 15 min,SPO_(2)decreased in the control group and was lower than that in the observation group(P<0.05);PCO_(2),mean arterial pressure,and heart rate decreased in both groups,but were higher in the observation group than in the control group(P<0.05).The incidence of adverse reactions in the observation group was 4.00%,which was lower than that in the control group(P<0.05).Conclusion:Propofol and remifentanil combined anesthesia and Dex and remifentanil combined anesthesia can play an ideal role in pediatric anesthesia to maintain laryngeal mask ventilation,but the latter can maintain the hemodynamic stability of the children,and the safety of anesthesia is better.
