基于数据挖掘研究偏头痛中医证素规律

Study on Law of TCM Syndrome Elements of Migraine Based on Data Mining

目的采用数据挖掘技术分析偏头痛病位、病性、证素的分布情况和组合规律,为临床诊疗提供参考。方法采用回顾性研究方法,收集昭通市中医医院2019年1月1日-2021年12月31日偏头痛患者相关信息,按照《证素辨证学》进行证素辨识。采用R4.1.2软件进行关联规则分析、聚类分析,利用Cytoscape3.9.0软件展示复杂网络,总结证素规律。结果纳入病例501例,男性患者121例,女性患者380例,男女比为1∶3.14;涉及中医证素19种,其中病位证素6种,包括肝、脾、经络、肾、脑、胆,偏头痛发作关键病位为肝、脾、肾;病性证素13种,包括血瘀、痰、阳亢、气虚、气滞、火(热)、阴虚、动风、寒、血虚、外风、湿、精亏;涉及65条关联规则,支持度排名靠前的组合为肝→阳亢、肝→血瘀、经络→血瘀、经络→痰、血瘀→经络;男性与女性偏头痛患者证素聚类存在差异,男性患者证素可聚为6类,女性患者证素可聚为5类。结论偏头痛核心证素为肝、经络、血瘀、痰、阳亢、气虚,且男女偏头痛证素组合规律不一致。

Objective To analyze the distribution and combination law of disease location,disease nature,and syndrome elements of migraine using data mining technology;To provide reference for clinical diagnosis and treatment.Methods A retrospective study method was used to collect information related to migraine patients in Zhaotong Hospital of Traditional Chinese Medicine from January 1,2019 to December 31,2021,and identify the syndrome elements in accordance with Zheng Su Bian Zheng Xue.R4.1.2 software was used for association rule analysis and clustering analysis,and Cytoscape 3.9.0 software was used to display the complex network and summarize the laws of syndrome elements.Results Totally 501 cases were included,121 male patients and 380 female patients,with a male-to-female ratio of 1:3.14;19 TCM syndrome elements were involved,including 6 pathogenic elements,including liver,spleen,meridian,kidney,brain and gallbladder,and the key pathogenic elements of migraine attacks were liver,spleen and kidney;13 pathogenic elements,including blood stasis,phlegm,hyperactivity of yang,qi deficiency,stagnation of qi,fire(heat),deficiency of yin,dynamic wind,cold,deficiency of blood.There were 65 association rules,and the top-ranked combinations were liver→hyperactivity,liver→blood stasis,meridian→blood stasis,meridian→phlegm,blood stasis→meridian;there were differences in the clustering of syndrome elements between male and female migraine patients,with male patients clustering into six categories and female patients clustering into five categories.Conclusion The core elements of migraine are liver,meridians,blood stasis,phlegm,hyperactivity of yang,and deficiency of qi,and the combination law of male and female migraine elements is inconsistent.
