角膜宽度与前房角参数在前房角组成上的相关分析

The correlation analysis of corneal width and anterior chamber angle parameters on anterior chamber angle composition

目的观察白内障患者手术前后角膜宽度和前房角参数的变化及分析它们之间的关系。方法回顾性临床研究。纳入2021年1月至2022年3月在本院就诊的白内障患者34例(43只眼),年龄(69.03±6.61)岁(60~86岁)。应用眼科超声生物显微镜(UBM)测量所有眼术前与术后3个月时的前房宽度(ACW)、中央前房深度(CACD)、瞳孔直径(PD)、小梁虹膜角度数(TIA)、房角开放距离(AOD)和小梁网睫状突距离(TCPD)。数据均采用配对t检验进行对比分析,ACW与其它参数的相关分析应用Pearson相关和线性回归分析。结果白内障手术后3个月时的ACW、CACD、TIA、AOD和TCPD数值与术前相比差异均有统计学意义,PD差异无统计学意义。手术前ACW与CACD、TIA、AOD均无相关性,与TCPD有负相关性。结论在前房角的变化上,角膜宽度与睫状体突位置在前房角的组成上并不完全独立,而是共同在前房角的组成上趋向形成一个相对稳定但呈动态变化的结构关系。

Objective To observe the changes in corneal width and anterior chamber angle parameters in cataract patients before and after cataract surgery and analyze the relationships among them.Methods A retrospective clinicalstudy.A total of 43 eyes of 34 patients(69.03±6.61 years old)were enrolled from January 2021 to March 2022 in Han Dan City Eye Hospital.The anterior chamber width(ACW),central anterior chamber depth(CACD),pupil diameter(PD),trabecular iris angle(TIA),angle opening distance(AOD)and trabecular-ciliary processes distance(TCPD)of all eyes were measured by ophthalmic ultrasound biomicroscopy(UBM)before and at 3 months after cataract surgery.These parameters were analyzed by paired t-test,and the pearson correlation and linear regression analysis were applied in the correlation analysis of anterior chamber width with other parameters.Results The differences of ACW,CACD,TIA,AOD and TCPD were statistically significant at 3 months after cataract surgery compared to those before cataract surgery.The difference between PD before and at 3 months after cataract surgery was not statistically significant.The ACW had no association with CACD,TIA or AOD,but had a negative association with TCPD before cataract surgery.The unilinear regression equation:ACW=13.044-1.959(TCPD),adjusted R2=0.113.Conclusions In the change of anterior chamber angle,the corneal width and the ciliary process position were not completely independent in the composition of anterior chamber angle.They together tended to form a relatively stable but dynamic structural relationship in the composition of anterior chamber angle.
