三维可视化重建技术在活体肾移植手术培训教学中的应用

摘要 目的:探讨应用三维可视化重建技术在住院医师活体肾移植手术培训教学中的作用及效果.方法:选取2019年1月至2022年1月西安交通大学第一附属医院肾移植科的46名住院医师(包括低年资住院医师、规范化培训医师、进修医师及专硕研究生)作为研究对象,随机分为试验组(23名)和对照组(23名).试验组:通过获取供者数字成像和通信数据进行三维可视化重建,并根据重建模型进行基础理论、制定手术方案或模拟手术的教学.对照组:使用传统手术教学模式,通过学习肾脏解剖、手术图谱、双肾动静脉CT血管造影(CT angiography,CTA)、输尿管CT造影(CT urography,CTU)及B超等进行教学.教学结束后对两组住院医师进行考核评分,比较两组在相关理论知识、手术操作及教学主观感受之间的差异.应用SPSS 26.0软件进行统计学分析.结果:试验组住院医师在理论知识掌握、学习兴趣、课堂表现、手术方案规划、培训满意度等方面相较于对照组表现更有优势,以上差异均具有统计学意义(P<0.05).此外,试验组住院医师的理论考核分数和实践考核分数亦比对照组分数高(89.26±4.13 vs 83.43±5.32,91.21±3.34 vs 85.13±4.66),以上差异均具有统计学意义(P<0.05).结论:应用三维可视化重建技术进行住院医师活体肾移植手术培训教学是一种新教学模式,可以显著提升培训质量和效率,增强理论知识的掌握和实践运用能力,能够取得良好的教学效果. Objective:To explore the role and effectiveness of 3D visualization reconstruction technology in the training of resident doctors for live kidney transplant surgery.Methods:Forty-six resident doctors(including junior residents,standardized trainees,advanced trainees,and postgraduate students)from the Department of Kidney Transplantation at Xian Jiaotong University First Affiliated Hospital,who participated from January 2019 to January 2022,were randomly assigned to an experimental group(23 participants)and a control group(23 participants).The experimental group utilized 3D visualization reconstruction based on donor digital imaging and communication data,and conducted teaching on basic theory,surgical planning,or simulated surgeries using the reconstruction model.The control group used traditional teaching methods,including learning through kidney anatomy,surgical atlases,CT angiography(CTA),CT urography(CTU),and ultrasound.After the training,assessments were conducted to compare the two groups in terms of theoretical knowledge,surgical skills,and subjective teaching satisfaction.Statistical analysis was performed using SPSS 26.0.Results:The experimental group demonstrated superior performance compared to the control group in terms of theoretical knowledge,learning interest,classroom performance,surgical planning,and training satisfaction,with all differences being statistically significant(P<0.05).Additionally,the experimental group achieved higher scores in theoretical and practical assessments(89.26±4.13 vs 83.43±5.32 and 91.21±3.34 vs 85.13±4.66),with these differences also being statistically significant(P<0.05).Conclusion:The application of 3D visualization reconstruction technology in training for live kidney transplant surgery significantly improves the quality and efficiency of training,enhances the mastery of theoretical knowledge and practical skills,and achieves good teaching results.
