摘要
目的分析慢性心力衰竭患者血清miR-217和miR-665表达与心功能和心肌重构的关系。方法选取2022年4月至2023年4月于内蒙古包钢医院收治的慢性心力衰竭患者60例作为研究组,另外选取同期正常体检的健康志愿者50例作为对照组。观察两组患者一般资料、miR-217、miR-665、心功能、心肌重构水平,分析miR-217、miR-665水平与心功能和心肌重构的相关性。结果与对照组相比,研究组miR-217、心输出量(CO)、左心室射血分数(LVEF)、左心室重构指数(LVRI)表达水平降低,miR-665、左心室舒张末期内径(LVEDD)、左心房内径(LAD)、左心室后壁厚度(LVPWT)、左心室重量指数(LVMI)表达水平升高(P<0.05)。与心功能Ⅰ~Ⅱ级相比,Ⅲ~Ⅳ级患者miR-217、LVRI表达水平降低,miR-665、LVPWT、LVMI表达水平升高(P<0.05);与心功能Ⅲ级患者相比,Ⅳ级患者miR-217、LVRI表达水平降低,miR-665、LVPWT、LVMI表达水平升高(P<0.05)。相关性分析显示,miR-217水平与CO、LVEF、LVRI呈负相关,与LVEDD、LAD、LVPWT、LVMI呈正相关;miR-665水平与CO、LVEF、LVRI呈正相关,与LVEDD、LAD、LVPWT、LVMI呈负相关(P<0.05)。结论在慢性心力衰竭患者体内血清miR-217表达升高,miR-665表达降低,且miR-217、miR-665表达水平与心功能和心肌重构相关,检测miR-217、miR-665表达水平可用于患者病情的评估。
Objective To analyze the relationship among serum expressions of serum microRNA-217(miR-217)and microRNA-665(miR-665),heart function and myocardial remodeling in patients with chronic heart failure(CHF).Methods CHF patients(n=60)were chosen into study group and healthy volunteers with normal physical examination(n=50)were chosen into control group from Baogang Hospital of Inner Mongolia from Apr.2022 to Apr.2023.The general materials were observed in 2 groups.The levels of miR-217 and miR-665,and indexes of heart function and myocardial remodeling were detected.The correlation among levels of miR-217 and miR-665,heart function and myocardial remodeling were analyzed.Results The expressions of miR-217,cardiac output(CO),left ventricular ejection fraction(LVEF)and left ventricular remodeling index(LVRI)decreased,and expressions of miR-665,left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAD)and left ventricular posterior wall thickness(LVPWT)increased in study group compared with control group(P<0.05).The expressions of miR-217 and LVRI decreased,and miR-665,LVPWT and LVMI increased in patients with grades I andⅡof heart function classification compared with those with gradesⅢandⅣ(P<0.05).The expressions of miR-217 and LVRI decreased,and miR-665,LVPWT and LVMI increased in patients with gradesⅢcompared with those with gradesⅣ(P<0.05).The results of correlation analysis showed that miR-217 level was negatively correlated to CO,LVEF and LVRI,and positively correlated to LVEDD,LAD,LVPWT and LVMI,and miR-665 was positively correlated to CO,LVEF and LVRI,and negatively correlated to LVEDD,LAD,LVPWT and LVMI(P<0.05).Conclusion The expression of serum miR-217 increases and miR-665 expression decreases in CHF patients,and expressions of miR-217 and miR-665 are correlated to heart function and myocardial remodeling.The detection of expressions of miR-217 and miR-665 can applied to assess CHF patients’condition.
