摘要
目的:探讨血清高迁移率族蛋白B1(HMGB1)、B细胞活化因子(BAFF)与过敏性鼻炎患者病情严重程度的相关性。方法:选取收治的过敏性鼻炎患者160例,根据病情严重程度分为轻中症组(104例)和重症组(56例)。比较两组血清HMGB1、BAFF水平。采用Logistic回归分析过敏性鼻炎患者病情严重程度的影响因素。采用Pearson法分析血清HMGB1、BAFF与免疫球蛋白E(IgE)水平的相关性,Spearman法分析血清HMGB1、BAFF水平与视觉模拟量表(VAS)及鼻症状总积分表(TNSS)的相关性。绘制受试者工作特征(ROC)曲线分析血清HMGB1、BAFF对过敏性鼻炎重症患者的诊断价值。结果:两组患者有无过敏性结膜炎、IgE水平、VAS评分及TNSS评分比较差异有统计学意义(均P<0.05)。与轻中症组比较,重症组血清HMGB1、BAFF水平升高(均P<0.05)。VAS评分、TNSS评分及血清IgE、HMGB1、BAFF是影响患者病情严重程度的独立影响因素(均P<0.05)。血清HMGB1、BAFF水平与患者IgE水平、VAS评分以及TNSS评分呈正相关(均P<0.05)。血清HMGB1和BAFF联合诊断的曲线下面积(AUC)高于HMGB1、BAFF单独诊断的AUC(均P<0.05)。结论:血清HMGB1、BAFF水平与过敏性鼻炎患者病情程度严重程度有关,两者联合对患者病情严重程度具有一定诊断价值。
Objective:To investigate the correlation between serum levels of high mobility group box 1(HMGB1)and B-cell activating factor(BAFF)and the severity of allergic rhinitis in patients.Methods:A total of 160 patients with allergic rhinitis were enrolled and categorized into a mild-to-moderate group(104 cases)and a severe group(56 cases)based on the severity of their condition.Serum levels of HMGB1 and BAFF were compared between the two groups.Logistic regression was used to analyze the factors influencing the severity of allergic rhinitis.The correlation between serum HMGB1,BAFF and immunoglobulin E(IgE)levels was analyzed by Pearson method,and the correlation between serum HMGB1,BAFF and Visual Analog Scale(VAS)and Total Nasal Symptom Score(TNSS)was analyzed by Spearman method.The diagnostic value of serum HMGB1 and BAFF for severe allergic rhinitis patients was analyzed using the receiver operating characteristic(ROC)curve.Results:There were significant differences in allergic conjunctivitis,IgE level,VAS score and TNSS score between the two groups(all P<0.05).Compared with mild-to-moderate group,serum HMGB1 and BAFF levels in severe group were increased(all P<0.05).VAS score,TNSS score,serum IgE,HMGB1 and BAFF were independent influencing factors on the severity of patients’condition(all P<0.05).Serum HMGB1 and BAFF levels were positively correlated with IgE level,VAS and TNSS scores(all P<0.05).The area under the curve(AUC)for the combined diagnosis of HMGB1 and BAFF was higher than that for individual diagnoses of HMGB1 or BAFF(all P<0.05).Conclusion:Serum levels of HMGB1 and BAFF are related with the severity of allergic rhinitis in patients,and their combined detection has certain diagnostic value for the severity of the patients’condition.
