微针导入贻贝粘蛋白在红斑-毛细血管扩张型玫瑰痤疮治疗中的疗效观察

Observation on the Clinical Therapeutic Effect of Microneedle with Mussel Adhesive Protein in Erythema-Telangiectasia Rosacea

摘要 目的观察微针导入贻贝粘蛋白治疗红斑-毛细血管扩张型玫瑰痤疮的临床疗效。方法收集2021年12月至2022年12月我院收治的58例稳定期红斑-毛细血管扩张型玫瑰痤疮患者,根据治疗方式分为微针导入组20例,强脉冲光组20例,药物对照组18例。贻贝粘蛋白组采用1.0 mm滚轮微针治疗,每月1次,共3次。强脉冲光组采用波长为560~950 nm强脉冲光治疗,每月1次,共3次。药物对照组采用外用0.03%他克莫司药膏治疗共3月。分别于治疗前及3次治疗后1个月通过VISIA检测患者面部红斑及毛细血管扩张皮损的消退情况,并由2名临床医师共同评估临床治疗效果。结果贻贝粘蛋白组:VISIA检测评分(65.73±3.6)%,临床治疗效果评估治愈4例,好转13例,无效3例,总有效率85.00%;强脉冲光组:VISIA检测评分(62.43±4.83)%,临床治疗效果评估治愈1例,好转13例,无效6例,总有效率70.00%;药物对照组:VISIA检测评分(49.59±2.53)%,临床治疗效果评估治愈2例,好转4例,无效12例,总有效率33.33%。3组的VISIA检测评分及临床治疗效果评估比较,差异均有统计学意义(P<0.05)。结论微针导入贻贝粘蛋白治疗红斑-毛细血管扩张型玫瑰痤疮较传统药物治疗及强脉冲光有效率高,面部红区数值改善明显,具有治疗效果好,不良反应轻的特点,值得临床推广使用。 Objective To investigate the clinical effect of microneedle with mussel adhesive protein(MAP)in erythema-telangiectasia rosacea(ETR).Methods From December 2021 to December 2022,58 patients with ETR treated in our hospital were divided into MAP group(20 cases),intense pulsed light(IPL)group(20 cases)and drug control group(18 cases)according to different treatments.MAP group was treated by microneedle(1.0 mm)once a month,three times in total.IPL group was treated with 560 nm~950 nm IPL once a month,three times in total.Drug control group was treated with 0.03%Tacrolimus for 3 months.Finally,VISIA was used to detect the regression of facial erythema and capillary dilatation of patients before treatment and 1 month after 3 treatments respectively,and the clinical treatment effect was jointly evaluated by 2 clinicians.Resutls In the MAP group,the VISIA score was(65.73±3.6)%,and 4 cases were cured,13 cases were improved,3 cases were ineffective,and the total effective rate was 85.00%.In the IPL group,the VISIA score was(62.43±4.83)%,and 1 cases was cured,13 cases were improved,6 cases were ineffective,and the total effective rate was 70.00%.In the drug control group,the VISIA score was(49.59±2.53),and 2 cases were cured,4 cases were improved,12 cases were ineffective,and the total effective rate was 33.33%.There were significant differences in the VISIA score and the clinical treatment effect between the three groups(P<0.05).Conclusion Compared with traditional drug treatment and IPL,microneedle injection of mussel mucin in the treatment of erythema-telangiectasia rosacea has a higher effective rate,and the value of facial red area has been significantly improved,which has the characteristics of good therapeutic effect and light adverse reactions,and is worthy of clinical promotion and use.
