基于Swanson关怀理论的人文干预对肺癌化疗患者负性情绪及依从性的影响

Influence of humanistic intervention based on Swanson’s caring theory on negative emotion and compliance in patients with lung cancer chemotherapy

目的探讨基于Swanson关怀理论的人文干预对肺癌化疗患者负性情绪及依从性的影响。方法将89例肺癌化疗患者按干预方式的不同分为对照组(44例)与观察组(45例)。对照组患者采取常规干预,观察组患者在对照组的基础上采取基于Swanson关怀理论的人文干预。比较两组患者的负性情绪[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分]、疼痛程度[视觉模拟评分法(VAS)评分]、不良反应、依从性及护理满意度。结果干预后,两组患者HAMA、HAMD、VAS评分均较干预前降低,且观察组患者HAMA、HAMD、VAS评分均低于对照组,差异均有统计学意义(P﹤0.05)。观察组患者总依从率、护理总满意度均高于对照组,不良反应总发生率低于对照组,差异均有统计学意义(P﹤0.05)。结论基于Swanson关怀理论的人文干预可改善肺癌化疗患者的负性情绪,缓解疼痛程度,提高依从性及护理满意度,减少不良反应发生。

Objective To investigate the influence of humanistic intervention based on Swanson’s caring theory on negative emotion and compliance in patients with lung cancer chemotherapy.Method A total of 89 patients with lung cancer were divided into control group(44 cases)and observation group(45 cases)according to different intervention methods.The control group was treated with routine intervention,and the observation group was treated with humanistic intervention based on Swanson’s caring theory on the basis of the control group.Negative emotions[Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD)scores],pain degree[visual analogue scale(VAS)score],adverse reactions,compliance and nursing satisfaction were compared between the two groups.Result After intervention,the scores of HAMA,HAMD and VAS in two groups were lower than those before intervention,and the scores of HAMA,HAMD and VAS in observation group were lower than those in control group,the differences were statistically significant(P<0.05).The total compliance rate and total nursing satisfaction in observation group were higher than those in control group,and the total incidence of adverse reactions was lower than that in control group,the differences were statistically significant(P<0.05).Conclusion The humanistic intervention based on Swanson’s caring theory can improve negative emotions,relieve pain,improve compliance and nursing satisfaction,and reduce adverse reactions in patients with lung cancer chemotherapy.
