AGGF1表达与肝细胞癌侵袭转移及预后的关系

Relationship between AGGF1 expression and invasion,metastasis and prognosis of hepatocellularcarcinoma

摘要 目的检测具有G-patch和FHA结构域1的血管生成因子(AGGF1)在肝细胞癌(HCC)癌组织中的表达,探讨其与HCC血管生成和侵袭转移的关系及其临床意义。方法选择黄石市中心医院和湖北省肿瘤医院2019年1月至2020年9月手术治疗的原发性HCC患者共90例,收集所有患者临床病理资料。采用免疫组化检测90例HCC患者癌组织中AGGF1、CD34表达,计算微血管密度(MVD),随访HCC患者三年。分析AGGF1与血清血管生成素-2(Ang-2)含量、MVD及临床病理参数、术后预后的关系。结果HCC癌组织中AGGF1表达阳性率和相对表达量、MVD值均显著高于相应癌旁组织(均P<0.05);癌组织中AGGF1相对表达量与MVD值呈正相关(r=0.716)。AGGF1阳性患者血清Ang-2含量、MVD均显著高于AGGF1阴性患者(均P<0.01)。HCC癌组织中AGGF1表达量与肿瘤分化程度、肿瘤大小、中国肝癌分期方案(CNLC)分期、淋巴结转移、血管侵犯、术后复发转移显著相关(均P<0.01)。Kaplan-Meier分析显示AGGF1高表达患者术后2、3年生存率显著低于低表达患者(均P<0.01)。多因素分析显示CNLC分期、淋巴结转移、血管侵犯、AGGF1表达、术后复发转移是HCC患者术后生存期的影响因素(均P<0.05)。结论AGGF1高表达可促进HCC血管生成,与其侵袭转移、不良预后显著相关,可成为HCC临床预后的潜在评价指标。 Objective To detect the expression of angiogenic factor(AGGF1)with G-patch and FHA domain 1 in hepatocellular carcinoma(HCC),and to investigate its relationship with HCC angiogenesis,invasion and metastasis and its clinical significance.Methods A total of 90 primary HCC patients were selected from the Huangshi Central Hospital and Hubei Province Cancer Hospital from January 2019 to September 2020.Clinicopathological data of all patients were collected.The expression of AGGF1 and CD34 in cancer tissues of 90 HCC patients was detected by immunohistochemistry,and microvascular density(MVD)was calculated.The patients were followed up for three years.The relationship between AGGF1 and serum angiopoietin-2(Ang-2)content,MVD,clinicopathological parameters and postoperative prognosis was analyzed.Results The positive rate,relative expression level and MVD value of AGGFI in HCC tissues were significantly higher than those in adjacent tissues(all P<0.05).The relative expression of AGGF1 in cancer tissues was positively correlated with the MVD value(r=0.716).Serum Ang-2 content and MVD in AGGF1-positive patients were significantly higher than those in AgGF1-negative patients(all P<0.01).ACCF1 expression in HCC tissues was significantly correlated with tumor differentiation,tumor size,China Liver Cancer(CNLC)staging,lymph node metastasis,vascular invasion,and postoperative recurrence and metastasis(all P<0.01).Kaplan-Meier analysis showed that the 2-and 3-year survival rates of patients with high AGGF1 expression were significantly lower than those of patients with low AGGFI expression(all P<0.01).Multivariate analysis showed that CNLC stage,lymph node metastasis,vascular invasion,ACGF1 expression,postoperative recurrence and metastasis were the influencing factors for postoperative survival of HCC patients(all P<0.05).Conclusions The high expression of ACGF1 can promote the angiogenesis of HCC,and is significantly correlated with invasion and metastasis and poor prognosis,which can be a potential evaluation indicator of HCC clinical prognosis.
