穴位按摩联合大黄穴位贴敷对肿瘤阿片类药物所致便秘患者排便情况的影响

Effect of acupressure combined with Rhubarb acupressure on defecation in patients with constipation due to oncologic opioids

目的探讨穴位按摩联合大黄穴位贴敷在肿瘤阿片类药物所致便秘患者护理中的应用价值。方法选取2023年1月至2024年1月贵州省人民医院肿瘤科收治的在使用阿片类药物后发生便秘的98例肿瘤患者作为研究对象,按照随机数字表法将其分为对照组(49例)和观察组(49例)后进行单盲研究。对照组行常规护理,观察组在对照组基础上给予穴位按摩联合大黄穴位贴敷。比较两组便秘改善效果,观察患者排便等症状改善情况,同时观察患者生存质量变化情况。结果观察组护理总有效率(91.84%)高于对照组(75.51%),差异有统计学意义(P<0.05)。护理后观察组的排便性状、排便费力、排便时间、排便频率、腹胀、排便不尽等症状积分及总积分低于对照组,差异有统计学意义(P<0.05)。护理后观察组患者生存质量评分低于对照组,差异有统计学意义(P<0.05)。结论穴位按摩联合大黄穴位贴敷在肿瘤阿片类药物所致便秘患者应用中,可有效改善患者排便情况,护理效果显著,有助于进一步提升患者生存质量。

Objective To investigate the value of acupoint massage combined with rhubarb acupoint application in nursing care of patients with constipation caused by tumor opioids.Methods A total of 98 tumor patients with constipation after us-ing opioids admitted to the Department of Oncology of Guizhou Provincial People's Hospital from January 2023 to January 2024 were selected as the study objects,and they were divided into the control group(49 cases)and the observation group(49 cases)according to random number table method for single-blind study.The control group was treated with conventional interventions,while the observation group was given acupressure combined with Rhubarb acupressure on the basis of the control group.The treatment effects of the two groups were evaluated and compared,and the improvement of symptoms such as defecation was observed,as well as the changes in the quality of life of the patients.The effect of constipation improve-ment was compared between the two groups,and the improvement of defecation and quality of life were observed.Results The total effective rate of the observation group(91.84%)was higher than that of the control group(75.51%),the difference was statistically significant(P<0.05).After treatment,the scores of symptoms such as defecation traits,defecation effort,defecation time,defecation frequency,abdominal distension,incomplete defecation and total scores in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).After treatment,the qual-ity of life score of observation group was lower than that of control group,and the difference was statistically significant(P<0.05).Conclusion Acupoint massage combined with rhubarb acupoint application in patients with constipation caused by tumor opioids can effectively improve the defecation of patients,the nursing effect is significant,and help to further improve the quality of life of patients.
