基于慢病管理模型构建的“互联网+”中医健康管理模式在缺血性脑卒中患者的应用效果

Application effect of"Internet+"traditional Chinese medicine health management model based on chronic disease management model in patients with ischemic stroke

目的探讨基于慢病管理模型构建的“互联网+”中医健康管理模式对缺血性脑卒中患者神经功能恢复、自我管理效能、生存质量的影响。方法选取2023年1月至6月南昌大学第一附属医院康复科的100例住院患者作为研究对象,采用随机数字表法将其分为试验组和对照组,每组各50例。对照组出院患者实施常规的随访,试验组出院患者实施基于慢病管理模型构建的“互联网+”中医健康管理模式。比较两组患者出院6个月后改良RanKin量表、慢性病患者自我效能量表、脑卒中专门化生存质量量表得分。结果试验组神经功能预后良好患者占比高于对照组,差异有统计学意义(P<0.05);试验组自我管理效能得分高于对照组,差异有统计学意义(P<0.05);试验组患者生存质量得分高于对照组,差异有统计学意义(P<0.05)。结论基于慢病管理模式构建的“互联网+”中医健康管理模式能够促进患者神经功能的恢复,提升患者自我管理效能,改善患者的生活质量,值得临床推广。

Objective To explore the effect of"Internet+"traditional Chinese medicine health management model based on chronic disease management model on neurological recovery,self-management efficacy and quality of life of patients with ischemic stroke.Methods A total of 100 inpatients in the Department of Rehabilitation,the First Affiliated Hospital of Nanchang University from January to June 2023 were selected as the research objects and they were divided into experimental group and control group by random number table method,with 50 cases in each group.The discharged patients in the control group received routine follow-up,and the discharged patients in the experimental group received the"Internet+"traditional Chinese medicine health management model based on the chronic disease management model.The scores of modified RanKin scale,patients'chronic disease self-efficacy scale and stroke-specialized quality of life scale were compared between the two groups 6 months after discharge.Results The proportion of patients with good neurological function prognosis in experimental group was higher than that in control group,and the difference was statistically significant(P<0.05).The score of self-management efficacy in experimental group was higher than that in control group,and the difference was statistically significant(P<0.05).The quality of life score of experimental group was higher than that of control group,and the difference was statistically significant(P<0.05).Conclusion The"Internet+"traditional Chinese medicine health management model based on chronic disease management model can promote the recovery of patients'neurological function,improve patients'self-management efficiency,and improve patients'quality of life,which is worthy of clinical promotion.
