摘要
目的分析自我决定理论应用于肝癌切除术患者中对其心理弹性及围术期综合征的影响。方法选择2019年1月至2023年9月普外肝胆外科接受诊疗的肝癌切除术患者210例,电脑随机数表法分为对照组和观察组,每组105例。对照组给予常规护理,观察组给予通过自我决定理论予以护理,比较2组患者的心理弹性、健康促进行为、围术期综合征及生存质量。结果观察组的心理弹性量表(CD-RISC)分数高于对照组(P<0.05)。观察组的健康促进生活方式量表(HPLP)分数高于对照组(P<0.05)。观察组的肝癌患者围术期症状负担评估量表(MDASI-Periop-Hep)分数低于对照组(P<0.05)。观察组的肝癌患者生存质量测定量表2.0版(QOL-LC V2.0)分数高于对照组(P<0.05)。结论自我决定理论应用于肝癌切除术患者中可取得显著效果,不仅能够改善患者的心理弹性,增强健康促进行为水平,而且还能改善其围术期综合征,促进患者生存质量升高。
Objective To analyze the effects of the nursing method based on self-determination theory(SDT)for psychological resilience and perioperative syndrome in patients underwent hepatectomy.Methods Two hundred and ten patients underwent hepatectomy who were treated in the Department of Hepatobiliary Surgery of the hospital from January 2019 to September 2023 were enrolled.They were randomised 1∶1 to receive routine nursing(control group)or the nursing method based on SDT(observation group)according to a computer-generated table.The psychological resilience,health-promoting behaviors,perioperative syndrome,quality of life were compared between the two groups.Results The observation group presented significantly higher Connor-Davidson Resilience Scale(CD-RISC)score and health-promoting lifestyle profile(HPLP)score,lower Anderson Symptom Inventory for hepatectomy perioperative care(MDASI-PeriOp-Hep)score,and higher Quality of Life-Liver Cancer V2.0(QOL-LC V2.0)score than the control group(P<0.05).Conclusion The nursing method based on SDT for patients underwent hepatectomy has significant effects can not only improve psychological resilience and enhance the level of health-promoting behaviors,but also improve perioperative symptom and the quality of life.
