阴道菌群多样性和代谢产物分析及与HPV感染的相关性

摘要 目的探讨人乳头瘤病毒(HPV)感染患者阴道菌群多样性和代谢产物并分析其相关性。方法选取2022年1月至2023年1月体检科或妇产科就诊的女性100例,采集患者宫颈分泌物检测其HPV感染情况,根据患者有无感染HPV,取阴道分泌物检测其阴道菌群及其代谢产物,明确其相关性。多因素Logistic回归分析影响HPV感染发生的因素。结果100例患者HPV检测显示,HPV阳性46例。HPV感染组细菌、菌群感染阳性占比、唾液酸苷酶、菌群多样性Ⅰ级占比及H_(2)O_(2)阳性占比均明显高于无HPV感染组(P<0.05),乳酸杆菌阳性占比、菌群多样性、密集性Ⅱ~Ⅲ级占比明显低于无HPV感染组(P<0.05),2组间霉菌、支原体、滴虫、混合菌阳性占比、菌群多样性、密集性Ⅳ级占比、白细胞脂酶、乙酰氨基葡萄糖苷酶阳性占比比较差异无统计学意义(P>0.05);多因素Logistic回归分析显示,菌群多样性、菌群密集性及H_(2)O_(2)为影响HPV感染的因素(P<0.05)。结论阴道菌群多样性、密集性、H_(2)O_(2)与HPV感染有关,可增加HPV感染风险。 Objective To investigate the correlation between diversity of vaginal microbiota and its metabolites with human papillomavirus(HPV)infection.Methods One hundred women who visited the department of physical examination or department of obstetrics and gynecology of our hospital from January 2022 to January 2023 were enrolled.The cervical secretions were collected to determine the presence or absence of HPV infection.The correlation between diversity of vaginal microbiota and its metabolites was determined by detecting vaginal secretions.The influencing factors of HPV infection was analyzed using multivariate logistic regression.Results There were 46 patients with positive HPV among 100 patients.The HPV infection group had significantly higher positive rates of bacteria,microbial community infection,sialidase,density of gradeⅠ,and hydrogen peroxide(H_(2)O_(2))compared to the non HPV infection group(P<0.05).whereas,the HPV infection group presented significantly lower positive rates of lactobacilli,microbial community diversity,and density of gradeⅡ/Ⅲrelative to the non HPV infection group(P<0.05).The differences in positive rates of mold,mycoplasma,trichomonas,and mixed bacteria,as well as positive proportion of microbial community diversity and density of gradeⅣ,leukocyte lipase,acetylglucosaminidase were not significant(P>0.05).Multivariate logistic regression analysis showed that microbial community diversity,microbial community density,and H_(2)O_(2)were the influencing factors of HPV infection(P<0.05).Conclusion microbial community diversity,microbial community density as well as H_(2)O_(2)are associated with HPV infection,and with a increase risk for HPV infection.
