宫腔镜下电切术与宫腔镜下刮宫术在异常子宫出血临床治疗中的作用分析

Clinical Analysis of Hysteroscopic Electrotomy and Hysteroscopic Curet⁃tage in the Treatment of Abnormal Uterine Bleeding

目的探讨不同术式(宫腔镜下电切术、宫腔镜下刮宫术)治疗异常子宫出血患者的临床应用。方法方便选择2021年1月—2022年6月江苏省睢宁县中医院收治的84例择期采取手术治疗的异常子宫出血患者为研究对象,依据不同治疗方法分为对照组和观察组,每组42例。对照组采用宫腔镜下刮宫术治疗,观察组采用宫腔镜下电切术治疗,对比两组患者手术指标、并发症发生率、治疗有效率及复发率。结果观察组手术时间、月经恢复时间和住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P均<0.05)。观察组并发症总发生率为4.76%(2/42),低于对照组的19.05%(8/42),差异有统计学意义(χ^(2)=4.087,P<0.05)。观察组治疗有效率高于对照组,差异有统计学意义(P<0.05)。术后3个月、6个月,两组患者术后复发率比较,差异无统计学意义(P均>0.05)。结论宫腔镜应用于异常子宫出血治疗具有创伤性小、疗效显著等优势,能够减少并发症的发生、促进术后恢复,且宫腔镜下电切术的应用价值更为显著。

Objective To investigate the clinical application of different surgical methods(hysteroscopic electrotomy and hysteroscopic curettage)in the treatment of patients with abnormal uterine bleeding.Methods From January 2021 to June 2022,84 patients with abnormal uterine bleeding undergoing elective surgical treatment in Suining Traditional Chinese Medicine Hospital in Jiangsu Province were conveniently selected as the study objects.According to different treatment methods,they were divided into control group and observation group,with 42 cases in each group.The con⁃trol group was treated with hysteroscopic curettage,and the observation group was treated with hysteroscopic elec⁃trotomy.The surgical indexes,complication rate,treatment effectiveness and recurrence rate were compared between the two groups.Results The operation time,menstrual recovery time and hospital stay in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,the dif⁃ferences were statistically significant(all P<0.05).The total incidence of complications in the observation group was 4.76%(2/42),lower than that in the control group 19.05%(8/42),and the difference was statistically significant(χ^(2)=4.087,P<0.05).The effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After 3 months and 6 months,there was no significant differ⁃ence in postoperative recurrence rate between the two groups(both P>0.05).Conclusion Hysteroscopy in the treat⁃ment of abnormal uterine bleeding has the advantages of less trauma and significant efficacy,which can reduce the oc⁃currence of complications and promote postoperative recovery,and the application value of hysteroscopic electrotomy is more significant.
