丙戊酸钠对女性癫痫患者甲状腺激素与性激素水平的影响

Effects of sodium valproate on the levels of thyroid and sex hormones in female epileptic patients

目的探讨丙戊酸钠对女性癫痫患者甲状腺激素与性激素水平的影响。方法选取40例服用丙戊酸钠的女性癫痫患者设为试验组,另选取同期未服用任何药物的40名健康女性设为对照组。采集两组受试者静脉血,测定促甲状腺激素(TSH)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、垂体泌乳素(PRL)、孕酮(PROG)、卵泡生成素(FSH)和雌二醇(E2)水平,并采用Pearson相关分析探讨女性癫痫患者甲状腺激素相关指标间的关系、性激素相关指标间的关系。结果试验组患者TSH、PRL水平高于对照组受试者(P<0.01),PROG、E2水平均低于对照组受试者(P<0.01)。试验组患者TSH与T4水平呈负相关(P<0.01),T3与FT3水平、T4与FT4水平、PROG与E2水平呈正相关(P<0.05或0.01)。结论丙戊酸钠会对女性癫痫患者甲状腺激素和性激素的水平产生显著影响,在临床用药期间应实时跟踪监测女性癫痫患者的甲状腺激素和性激素水平并根据实际情况予以干预和治疗,以减轻药物对患者产生的副作用。

Objective To explore the effects of sodium valproate on the levels of thyroid and sex hormones in female epileptic patients.Methods Forty female epileptic patients on sodium valproate were selected as experimental group and 40 drug-free healthy women in the same term as control group.Venous blood was collected f rom both groups to measure the thyroid-stimulating hormone(TSH),triiodothyronine(T3),thyroxine(T4),free triiodothyronine(FT3),free thyroxine(FT4),pituitary prolactin(PRL),progesterone(PROG),follicle-stimulating hormone(FSH),and estradiol(E2)levels,Pearson correlation analysis used to explore the relationships among thyroid-hormones related indicators and relationships among sex-hormones related indicators in female epileptic patients.Results TSH and PRL level were higher and PROG and E2 lower in exper imental than cont rol group(P<0.01).TSH and T4 levels were negatively correlated(P<0.01),T3 and FT3,T4 and FT4,PROG and E2 positively correlated(P<0.05 or 0.01)in experimental group.Conclusion Sodium valproate has significant effects on thyroid and sex hormones,the levels of thyroid and sex hormones should be realtimely traced and monitored in clinical practice,and intervention and treatment be performed according to actual situation to alleviate side effect of drugs.
