中医五音疗法联合耳穴揿针对肺结节患者焦虑抑郁状况和睡眠质量的疗效分析

Curative effect of TCM five-tone therapy plus auricular points thumb-tack needle on anxiety,depression and sleep quality in patients with pulmonary nodules

目的探讨中医五音疗法联合耳穴揿针对初诊肺结节(PN)患者焦虑、抑郁状况和睡眠质量的影响及疗效分析。方法将60例初诊PN患者作为研究对象,将其按照数字奇偶法分为对照组(个体化常规治疗,30例)和研究组(个体化常规治疗+中医五音疗法+耳穴揿针,30例)。对比两组患者临床症状评分、临床疗效、胸部CT影像学结果、不良反应发生情况及焦虑自评量表(SAS)、抑郁自评量表(SDS)、匹兹堡睡眠质量指数(PSQI)评分。结果治疗后,两组患者主症、次症评分及总分均低于治疗前,且研究组低于对照组(P<0.01)。研究组患者临床疗效总有效率高于对照组(P<0.05)。治疗后,两组患者SAS、SDS评分均低于治疗前,且研究组低于对照组(P<0.01);两组患者PSQI各维度评分均低于治疗前,且研究组低于对照组(P<0.05或0.01);两组患者结节数量、结节直径均优于治疗前,且研究组优于对照组(P<0.05或0.01)。两组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。结论中医五音疗法联合耳穴揿针治疗初诊PN患者,可缓解焦虑、抑郁状况,改善临床症状及睡眠质量,提高临床疗效,且安全可靠,值得在临床中进行推广应用。

Objective To explore the curative effect of TCM five-tone therapy(TCM-FTT)plus auricular points thumb-tack needle(APTTN)on anxiety,depression and sleep quality in patients with pulmonary nodules.Methods Sixty patients with preliminary diagnosis of pulmonary nodules were selected research objects and divided into control(n=30,treated with individualized routine treatment)and research group(n=30,done with routine treatment+TCM-FTT+APTTN)according to digital parity method.Such indexes were compared between two groups as clinical symptoms scores,clinical effects,results of chest CT imaging,adverse reactions as well as scores on Self-raring Anxiety Scale(SAS),Self-rating Depression Scale(SDS)and Pittsburgh Sleep Quality Index.Results After treatment,the main symptom,secondary symptom and total scores were lower in both groups compared with pretreatment and so were those in research than control group(P<0.01).The total effective rate was higher in research than control group(P<0.05).After tratment,scores on SAS and SDS in both groups were lower compared with pre-treatment and so were those in research than control group(P<0.01).After treatment,the PSQI scores were lower in both groups compared with pre-treatment and so were those in research than control group(P<0.05or 0.01).The numbers and diameters of nodules were better in both groups compared with pretreatment and so were those in research than control group(P<0.05 or 0.01).Intergroup difference in the total incidence of adverse reactions wasn't statistically significant(P>0.05).Conclusion The TCMFTT plus APTTN could improve clinical symptoms and sleep quality of patients with preliminary diagnosis of pulmonary nodules,alleviate anxious and depressive emotions,improve clinical effect,is safe and reliable,and deserves clinical promotion and application.
