肾盂输尿管肿瘤治疗中应用后腹腔镜联合经尿道电切镜的安全性

Efficacy and Safety of Retroperitoneal Laparoscopy Combined with Transurethral Resection in the Treatment of Renal Pelvic and Ureteral Tumors

目的研究腹腔镜+经尿道电切镜治疗肾盂输尿管肿瘤的效果及安全性。方法选取滕州市中心人民医院泌尿外科于2020年5月—2022年12月收治的50例肾盂输尿管肿瘤患者为研究对象,按照手术方法不同将患者分为对照组(25例,开放手术)与联合组(25例,腹腔镜+经尿道电切镜),对比两组手术指标、临床症状(尿血、绞痛)、恢复时间(引流管拔出时间、肠功能恢复时间、下床活动时间、住院时间)、疼痛状况、并发症发生率。结果联合组并发症发生率为0,低于对照组的16.00%(4/25),差异有统计学意义(P=0.037)。两组手术时间比较,差异无统计学意义(P>0.05)。联合组出血量少于对照组,差异有统计学意义(P<0.05)。治疗后,联合组临床症状评分、疼痛评分、恢复时间均优于对照组,差异有统计学意义(P均<0.05)。结论肾盂输尿管肿瘤治疗过程中采用腹腔镜联合经尿道电切镜效果好,可以有效改善患者病情,降低手术损伤,保障手术安全性,恢复时间更短。

Objective To study the efficacy and safety of laparoscopy combined with transurethral resection in the treatment of renal pelvic and ureteral tumors.Methods A total of 50 patients with renal pelvic and ureteral tumors admitted to the Department of Urology of Tengzhou Central People's Hospital from May 2020 to December 2022 were selected as the research objects.According to different surgical methods,the patients were divided into control group(25 cases,open surgery)and combined group(25 cases,laparoscopy+transurethral resection).The operation indexes,clinical symptoms score(hematuria,colic),recovery time(drainage tube extraction time,intestinal function recovery time,ambulation time,hospitalization time),pain status and incidence of complications were compared between the two groups.Results The incidence of complications in the combined group was 0,which was lower than 16.00%(4/25)in the control group,and the difference was statistically significant(P=0.037).There was no significant difference in operation time between the two groups(P>0.05).The bleeding volume in the combined group was less than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the clinical symptom score,pain score and recovery time of the combined group were better than those of the control group,and the differences were statistically significant(all P<0.05).Conclusion Laparoscopic combined with transurethral resection was effective in the treatment of renal pelvic and ureteral tumors,whic can effectively improve the patient's condition,reduce the surgical injury,ensure the safety of the operation,and the recovery time was shorter.
