低剂量舒芬太尼联合右美托咪定静脉自控镇痛在下肢骨折术后镇痛中的应用探讨

Exploring the Application of Low-dose Sufentanil Combined with Dexmedetomidine Intravenous Patient-controlled Analgesia in Postoperative Pain Management of Fractures

目的分析下肢骨折患者术后应用低剂量舒芬太尼联合右美托咪定静脉自控镇痛的效果。方法非随机选取2022年5月—2023年9月相城人民医院收治的下肢骨折患者86例为研究对象。按术后镇痛方法分为两组,每组43例。对照组给予常规剂量舒芬太尼联合右美托咪定静脉自控镇痛;观察组给予低剂量舒芬太尼联合右美托咪定静脉自控镇痛。对比两组患者静脉自控镇痛按压次数以及麻醉药用量、不良反应发生情况。结果观察组静脉自控镇痛按压次数、麻醉药用量均低于对照组,差异有统计学意义(P均<0.05)。观察组不良反应总发生率为4.65%(2/43),低于对照组的18.60%(8/43),差异有统计学意义(χ^(2)=4.074,P<0.05)。结论下肢骨折患者术后应用低剂量舒芬太尼复合右美托咪定静脉自控镇痛,麻醉药用量少,且不良反应少。

Objective To analyze the effect of postoperative application of low-dose sufentanil combined with dexmedetomidine for intravenous self-controlled analgesia in patients with lower extremity fractures.Methods Eighty-six patients with lower limb fractures admitted to Xiangcheng People's Hospital from May 2022 to September 2023 were non-randomly selected as study subjects.They were divided into two groups according to the method of postoperative analgesia,with forty-three cases in each group.The control group was given conventional dose sufentanil combined with dexmedetomidine intravenous self-controlled analgesia;the observation group was given low-dose sufentanil combined with dexmedetomidine intravenous self-controlled analgesia.Compared the number of times of intravenous self-controlled analgesia pressure,as well as the dosage of anesthesia drugs and the occurrence of adverse reactions in the two groups of patients.Results The number of intravenous self-controlled analgesic presses and the dosage of anesthetics in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The total incidence of adverse reactions in the observation group was 4.65%(2/43),which was lower than that of 18.60%(8/43)in the control group,and the difference was statistically significant(χ^(2)=4.074,P<0.05).Conclusion Postoperative application of low-dose sufentanil compounded with dexmedetomidine for intravenous self-controlled analgesia in patients with lower extremity fractures is associated with a low dosage of anesthetics and fewer adverse effects.
