带状疱疹病毒感染患者IL-17、IFN-γ/IL-4及其对疾病程度的预测价值

目的探讨带状疱疹病毒感染患者血清白细胞介素-17(IL-17)、干扰素γ(IFN-γ)/IL-4及其对疾病程度的预测价值。方法回顾性选取2022年12月-2023年12月丽水市中心医院收治的88例带状疱疹病毒感染患者作为研究组,另选取同期健康体检者50名作为对照组,统计分析两组血清IL-17、IFN-γ/IL-4水平;根据患者疾病程度将研究组分为轻中度组(n=21)和重度组(n=67),统计IL-17、IFN-γ/IL-4水平,采用受试者工作特征(ROC)曲线评估IL-17和IFN-γ/IL-4对带状疱疹病毒感染患者疾病程度的预测价值。结果果研究组血清IL-17、IL-4分别为(7.14±1.72)、(6.16±1.27)pgml高于对照组,IFN-γ、IFN-γ/IL-4分别为(1.32±0.37)pg/ml、0.23±0.06低于对照组(P<0.05);重度组血清IL-17、IL-4高于轻中度组,IFN-γ、IFN-γ/IL-4低于轻中度组(P<0.05);ROC曲线分析显示,IL-17、IFN-γ/IL-4联合预测带状疱疹病毒感染患者的曲线下面积(AUC)为0.904.高于IL-17的0.729、IFN-γ/IL-4的0.810(P<0.05);血清IL-17>7.66pg/ml、IFN-γ/IL-4≤0.21均为带状疱疹病毒感染患者疾病程度的影响因素(P<0.05)。结论带状疱疹病毒感染患者血清IL-17水平升高,IFN-γ/IL-4水平降低,二者联合对疾病程度具有一定的预测价值。

OBJECTIVE To explore serum interleukin-17(IL-17),interferon(IFN-γ)/interleukin-4(IL-4)in patients with herpes zoster virus infection and their predictive value for disease degree.METHODS A total of 88 patients with herpes zoster virus infection admitted to Lishui Central Hospital between Mar.2022 and Dec.2023 were retrospectively selected as the study group,and 5o healthy medical checkups during the same period were selected as the control group,and serum IL-17 and IFN-γ/IL-4 levels were statistically analyzed in the two groups.According to disease severity,patients in study group were divided into the mild-to-moderate group(n=21)and the severe group(n=67),the levels of IL-17 and IFN-γ/IL-4 were statistically analyzed,and the predictive value of IL-17 and IFN-γ/IL-4 on the degree of disease in patients with herpes zoster virus infection was assessed using the receiver operator characteristics(ROC)curve.RESULTS The levels of serum IL-17 and IL-4 in the study group were(7.14±1.72)and(6.16±1.27)pg/ml,higher than those in the control group,while levels of IFN-γ and IFN-γ/IL-4 were(1.32±0.37)pg/ml and 0.23±0.06.lower than those in the control group(P<0.05).The levels of serum IL-17 and IL-4 in the severe group were higher than those in the mild-to-moderate group,while levels of IFN-γ and IFN-γ/IL-4 were lower than those in the mild-to-moderate group(P<0.05).ROC curves analysis showed that AUC of IL-17 combined with IFN-γ/IL-4 for predicting herpes zoster virus infection was 0.904,which was higher than that of IL-17(0.729)and IFN-γ/IL-4(0.810)single detection.Serum IL-17>7.66 pg/ml and IFN-γ/IL-4≤0.21 were the influencing factors of disease severity in patients with herpes zoster virus infection.CONCLUSION The serum increased and decreased in Patients with herpes zoster virus infection had elevated serum IL-17 levels and reduced IFN-γ/IL-4 levels,and the combination of the two had a predictive value for the extent of disease.
