摘要
目的探讨2型糖尿病患者周围神经电生理指标与糖化血红蛋白(HbA1c)、血脂、体质量指数、糖尿病病程及高血压等因素之间的相关性。方法选取2010年4月至2015年9月大连医科大学附属第二医院内分泌科收治的92例2型糖尿病患者,并对他们连续随访4年,随访过程中收集患者的神经电生理指标、HbA1c、血脂、糖尿病病程及高血压等指标。结果连续随访4年后,发现92例2型糖尿病患者周围神经电生理指标的变化分布为(-128.96±63.44)%,这种变化与平均HbA1c水平间存在负相关(r=-0.652,P<0.001),而与糖尿病病程(r=0.117,P=0.267)、低密度脂蛋白胆固醇(r=-0.097,P=0.359)、高血压(t=-1.297,P=0.198)及糖尿病视网膜病变(t=1.945,P=0.055)等均无相关性。结论观察时间内仅平均HbA1c水平与2型糖尿病患者的周围神经电生理指标的变化之间存在显著的负相关,这表明平均HbA1c水平越高,神经电生理指标的恶化越明显。
Objective To investigate the correlations between peripheral neuroelectrophysiologic parameters of patients with type 2 diabetes and factors such as glycosylated hemoglobin(HbA1c),serum lipid,body mass index,duration of diabetes and hypertension.Methods A total of 92 inpatients with type 2 diabetes at the Department of Endocrine,the Second Affiliated Hospital of Dalian Medical University from April 2010 to September 2015.Follow-up lasted for four consecutive years.During the follow-up,the patients'neuroelectrophysiological indicators,HbA1c,blood lipids,diabetes duration,hypertension and other indicators were collected.Results After 4 years of continuous follow-up,the distribution of changes in peripheral neuroelectrophysiological indicators in 92 patients with type 2 diabetes was(-128.96±63.44)%.This change was negatively correlated with the average HbA1c level(r=-0.652,P<0.001),but not correlated with the course of diabetes(r=0.117,P=0.267),low-density lipoprotein cholesterol(r=-0.097,P=0.359),hypertension(t=-1.297,P=0.198)and diabetes retinopathy(t=1.945,P=0.055).Conclusion During the observation period,only the average HbA1c level has a significantly negative correlation with the changes of peripheral neuroelectrophysiological indicators of patients with type 2 diabetes,indicating that the higher the average glycosylated hemoglobin level,the more obvious the deterioration of the neuroelectrophysiological indicators.
