摘要
目的探究鼻塞式气道通气联合盐酸氨溴索对重症新生儿肺炎的治疗效果。方法选取2022年5月至2023年12月入住商丘市第一人民医院新生儿重症监护室的72例重症新生儿肺炎患者作为研究对象,随机分为对照组(鼻塞式气道通气)与研究组(鼻塞式气道通气+盐酸氨溴索),各36例。比较两组治疗效果。结果研究组发热、咳嗽、气喘、湿啰音等症状消失时间均低于对照组,差异有统计学意义(P<0.05)。治疗前,两组炎症指标差异无统计学意义(P>0.05);治疗后,研究组白细胞介素-2、C反应蛋白、降钙素原水平均低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血气指标差异无统计学意义(P>0.05);治疗后,研究组血氧分压、氧合指数高于对照组,二氧化碳分压低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论鼻塞式气道通气联合盐酸氨溴索治疗重症新生儿肺炎,可促使相关症状尽快消失,有效抑制机体炎症反应,改善机体血气指标,且不会额外增加不良反应。
Objective To explore the therapeutic effect of nasal airway ventilation combined with ambroxol hydrochloride in the treatment of severe neonatal pneumonia.Methods Seventy-two patients with severe neonatal pneumonia admitted to the Neonatal Intensive Care Unit of Shangqiu First People's Hospital from May 2022 to December 2023 were randomly divided into a control group(nasal airway ventilation)and a study group(nasal airway ventilation+ambroxol hydrochloride),with 36 patients in each group.The treatment effects of the two groups were compared.Results The study group had shorter symptom disappearance times for fever,cough,wheezing,and moist rales compared to the control group,with statistically significant differences(P<0.05).Before treatment,there were no significant differences in inflammation indicators between the two groups(P>0.05).After treatment,the levels of interleukin-2,C-reactive protein,and procalcitonin were lower in the study group than in the control group,with statistically significant differences(P<0.05).There were no significant differences in blood gas indicators between the two groups before treatment(P>0.05).After treatment,the study group showed higher partial pressure of oxygen and oxygenation index,and lower partial pressure of carbon dioxide compared to the control group,with statistically significant differences(P<0.05).The incidence of adverse reactions was not significantly different between the two groups(P>0.05).Conclusion Nasal airway ventilation combined with ambroxol hydrochloride in the treatment of severe neonatal pneumonia can promote the rapid disappearance of symptoms,effectively inhibit inflammatory response,improve blood gas indicators,and does not increase adverse reactions.
